鼻内窥镜下腺样体低温消融术联合鼓膜CO2激光打孔治疗分泌性中耳炎患儿的短期随访研究

目的:探究鼻内窥镜下腺样体低温消融术联合鼓膜CO 2激光打孔治疗分泌性中耳炎患儿的短期随访结果。方法:将84例(84耳)分泌性中耳炎患儿根据手术方案分组,各42例(42耳)。两组均接受鼻内窥镜下腺样体低温消融术,对照组给予鼓膜置管术治疗,实验组给予鼓膜CO 2激光打孔治疗,观察两组临床疗效、感染发生率,并于术后随访3个月,统计两组复发率。结果:实验组治疗总有效率为95.24%,高于对照组的73.81%(P<0.05);实验组感染发生率2.38%较对照组19.04%低(P<0.05);术后随访3个月,两组均无脱落病例,实验组复发率2.38%与对照组7.14%比较,无显著差异(P>0.05)。结论:鼻内窥镜下腺样体低温消融术联合鼓膜CO 2激光打孔治疗分泌性中耳炎患儿,可提高治疗效果,降低感染发生率,且在减少术后复发风险方面具有一定价值。

A Short-Term Follow-Up Study of Endoscopic Adenoids Cryoablation Combined with Tympanic Membrane CO_2 Laser Drilling in Children with Secretory Otitis Media

Objective:To investigate the short-term follow-up results of endoscopic adenoids cryoablation combined with tympanic membrane CO 2 laser drilling in children with secretory otitis media.Methods:84 children(84 ears)with secretory otitis media were divided into two groups according to the operation plan,with 42 cases(42 ears)in each group.Both groups were treated by endoscopic cryoablation of adenoids,while the control group was treated by tympanic membrane catheterization,and the experimental group was treated by CO 2 laser drilling of tympanic membrane.The clinical efficacy and incidence of infection in the 2 groups were observed,and the recurrence rate of the 2 groups was analyzed after 3 months follow-up.Results:The total effective rate of the experimental group was 95.24%,which was higher than that of the control group(73.81%,P<0.05);The infection rate of the experimental group was 2.38%lower than that of the control group 19.04%(P<0.05);Postoperative follow-up was conducted for 3 months,and there were no shedding cases in both groups.The recurrence rate of 2.38%in the experimental group and 7.14%in the control group showed no significant difference(P>0.05).Conclusion:Endoscopic adenoids cryoablation combined with tympanic membrane CO 2 laser drilling in the treatment of children with secretory otitis media can improve the therapeutic effect,reduce the incidence of infection,and has a certain value in reducing the risk of postoperative recurrence.
