临床实习医师对内科学毕业实习临床实践考试方法改革的态度研究

目的了解临床实习医师对“考站式”临床实践考试形式的态度,评价该考试方法的效果。方法自拟《内科学实习临床实践考试改革调查表》,对2005年期间参加考试的220名临床实习医师进行问卷调查。结果在各考站考试能否考核出学生的真实水平的调查中,应答者的肯定率为病史问诊和病例分析站76.4%,胸片和心电图站82.1%,体检站73.8%。68.2%的人认为病史问诊和病例分析考核评分公正,23.2%持否定态度;71.8%应答者认为胸片和心电图考核评分公正;69.8%认为体检考核评分公正。在考试合理性方面,应答者中79.9%的人赞成病史问诊和病例分析采用口试,59.2%赞成胸片和心电图考试采用现场判读形式,77.1%赞成体检采用人体实际操作。结论临床实习医师对“考站式”方法持肯定态度,这一考试方式基本能够公正评估医学生的临床实践能力。用“考站式”方法代替传统的床边考试形式,作为一种临床技能评估新方式是可行的。

The Study of Interns'''' Attitudes to the Improvement of Internal Medicine Clinical Examination

Objective To know and get feedback the interns' attitudes to site-style internal medicine clinical examination,and also evaluate the effect of that examination.Methods A questionnaire of interns' attitudes to the examination was made.We surveyed 220 interns by the questionnaire in written form who had taken part in the site-style examination in 2005.Results When the responders answered the question whether the examination could check the clinical acute level of the interns,the affirmation percentages of three sites were inquiring site(76.4%),chest and ECG site(82.1%),physical examination site(73.8%).And the percentages of the responders who affirmed the justice of the score were inquiring site(68.2%),chest and ECG site(71.8%),physical examination site(69.8%),but 23.2%of the responders denied justice in inquiring site.The percentages of the responders who affirmed the form of the exam were inquiring site(79.9%),chest and ECG site(59.2%),physical examination site(77.1%).Conclusions The attitudes of 220 interns' to site-style of internal medicine clinical examination are affirmative.The examination could evaluate clinical acute level of the interns.It is feasible that we adopt the site-style examination,which taking the place of the traditional style,to evaluate the clinic skills of the interns.
