显微手术治疗胼胝体前部胶质瘤30例报告

摘    要:目的:探讨胼胝体前部胶质瘤的临床特点、应用显微手术切除的可行性及其预后。方法:对我院1998年1月至2004年6月收治的30例经手术及病理证实的胼胝体前部胶质瘤患者进行回顾性分析。结果:手术全切除18例,次全切除9例,部分切除3例。术后无1例死亡,4例术后出现短暂性失语和反应迟钝,3例术后癫痫小发作,2例术后出现短暂性右上肢无力,21例术后恢复良好。22例随访6个月至6年,平均生存期18个月。结论:胼胝体前部胶质瘤的治疗以手术切除为首选,对于大部分病例,全切或次全切除肿瘤取得良好的临床效果是完全可能的。经胼胝体前部人路是治疗该区肿瘤的一个安全可行的选择。

Abstract:Objective: To study the clinical characteristics, microsurgical feasibility and prognosis of gliomas in the anterior corpus callosum. Methods: 30 cases treated with microsurgical technologies for gliomas in anterior corpus callosum were analyzed retrospectively in our hospital from January 1998 to June 2004. Results: Total removal was achieved in.18 cases, subtotal removal in 9 cases and biopsy in 3 cases. There were no operative deaths. The fine postoperative recovery resulted in 21 cases, but there were transient aphasia and reaction dullness in 4 cases, minor epilepsy in 3 cases and right upper extremities asthenia in 2 cases. The median survival time was 18 weeks in 22 cases followed up from six months to six years after operation. Conclusion: Microneurosurgical removal is the primary choice to treat gliomas in the anterior corpus callosum and anterior transcallosal approach is a safe and feasible alternative in management of tumors in this region.
