针灸所致椎管内硬膜外血肿的诊断及治疗

目的:探讨针灸治疗所致的椎管内硬膜外血肿的诊断及治疗措施.方法:对我院2000年3月～2005年1月收治的来自外院针灸治疗所致的7例椎管内硬膜外血肿患者进行回顾性分析.结果:本组7例患者行椎板减压后分别选用相应的的手术方法治疗,术后病理检查均证实为椎管内硬膜外血肿.血肿位于硬膜外腔的侧后方或正后方,其中发生于颈段3例、胸段2例、颈胸交界部1例、胸腰段1例.结论:针灸所致椎管内硬膜外血肿临床罕见,椎管内硬膜外血肿的MRI表现较具特征性,MRI可对其作出正确诊断,并指导手术,手术是治疗椎管内硬膜外血肿的有效方法.

Diagnosis and treatment of spinal epidural hematoma caused by acupuncture and moxibustion

Objective:To evaluate the diagnosis and treatment of spinal epidural hematoma caused by acupuncture and moxibustion.Methods:From March 2000 to January 2005,seven patients with spinal epidural hematoma caused by acupuncture and moxibustion from other hospitals were treated in our institution and the data ware analyzed retrospectively.Results:After decompression laminectomy of the sacral canal,the spinal epidural hematomas were treated with their corresponding operations,the post-operative pathologies of 7 cases were spinal epidural hematoma in the sacral canal.The hematomas were located in lateral posterior or posterior epidural space,including 3 cases in cervical vertebra,2 cases in thoracic vertebra,1 cases in cervical and thoracic junction region,and 1 case in thoracic and lumbar part.Conclusion:Clinically,spinal epidural hematoma caused by acupuncture and moxibustion was rare. and possesed characteristic in MRI.The correct diagnosis could be mde with MRI,which we also put it for directing the operations.Operation was substantiated as the effective method in the treatment of spinal epidural hematoma in the sacral canal.
