妇科腹腔镜手术180例临床分析

摘    要:目的:总结妇科腹腔镜手术的临床应用价值。方法:回顾分析我院180例腹腔镜手术的经验,异位妊娠术式为输卵管切开取胚术、输卵管切除术;卵巢囊肿手术采用囊肿剥除术、患侧附件切除术;子宫手术采用肌瘤剔除术、子宫次全切除术、子宫全切术及腹腔镜辅助阴式子宫切除术。结果:腹腔镜手术成功率96.11%,中转开腹率3.89%,并发症发生率0.56%,为盆腔血肿1例。结论:根据循序渐进的熟练操作技术,以进行相应手术,可获满意治疗效果并可望安全有效的开展妇科腹腔镜手术。

Abstract:Objective:To explore the value of laparoscopic operation in the treatment of gynecologic diseases.Methods:Proper application of the technical skills was described on the basis of 180 operations performed laparoscopically.Patients with ectopic pregnancyunderwent laparoscopic salp-ingotomy,salpingectomy or embryo remove from fimbria.For ovariancyst,oophorocystectomy adnexectomy.Improved LM(laparoscopic myomectomy),LSH(laparoscopic supracervical hysterectomy),LTH(lapar-oscopic total hysterecotomy),LAVH(laparoscopically assisted vagina hyst-erectomy) were performed for uterine problems.Results:173 cases were performed laparoscopically, with the success rate of 96.11%.7cases were converted to laparotomy,the rate being 3.89%. 1 case showed complications,the rate being 0.56%,1 pelvic hematoma.Conclusion: Mastery of operative laparoscopic techniques step by step along with the degree of surgical difficulty is madatory,and can deliver optimal clinical outcome hopefully.It will also help to expand the use of operative gynecologic laparoscopy in a safe and effective way.
