母婴皮肤接触对减轻足月新生儿足跟采血时疼痛的作用

摘    要:目的探讨母婴皮肤接触对减轻足月新生儿足跟采血时疼痛的作用。方法将40例足月新生儿分为两组,干预组足跟采血前20 min母亲与新生儿实施袋鼠式护理,整个采血过程及采血恢复期持续皮肤接触;对照组在常规下进行足跟血采集。比较两组新生儿在心率、血氧饱和度、面部疼痛表情持续时间及啼哭时间等方面的差异。结果从穿刺开始后10秒至结束后10秒7个时间段中,两组新生儿心率均有不同程度的改变,在足跟采血时间因素、交互作用和分组因素上有统计学意义(P<0.01);两组新生儿血氧饱和度之间存在统计学差异(P<0.05);两组新生儿疼痛表情持续时间、啼哭持续时间和新生儿急性疼痛行为评分量表得分差异均具有统计学意义(P<0.01)。结论母婴皮肤接触降低新生儿足跟采血时的疼痛感;减少新生儿足跟采血时心率变化;提高新生儿足跟采血时血氧饱和度。

Abstract:Objective To evaluate the effect of skin - to - skin contact relieving pain during heel blood collection in the newborn. Methods A total of 40 newborns were recruited and divided into the experimental group and the control group. Kangaro care was implemented in the 20 minutes before heel blood collection in the experimental group, and skin contact were continued during blood collection and recovery process. Routine blood collection method was employed in the control group. The difference of heart rate,oxygen saturation, duration of facial expressions of pain and crying time were compared between the two groups. Results The two groups had different change in heart rates and the main effect of time, group and interaction between time and group during the 7 time periods from 10 seconds after the beginning of puncture to 10 seconds after puncture were statistically significant (P 〈 0.01 ). The difference of the oxygen saturation between the two groups of newborn was statistically significant ( P 〈 0.05 ). There were significant differences of duration of facial expression of pain, crying time and the scores of the Douleur Aigne Nouveaune in experimental group ( P 〈 0.01 ). Conclusion Skin - to - skin contact can relieve pain during heel blood collection, reduce the heart rate change and increase the heel blood oxygen saturation.
