烧冲复合伤合并海水浸泡对犬血管内皮细胞的影响

摘    要:目的 探讨烧冲复合伤合并海水浸泡对血管内皮细胞影响的原因和机制 ,为海水浸泡伤早期救治提供一定的理论依据。方法 建立烧冲复合伤的犬实验模型 ,致伤后将犬随机分为非浸泡组和浸泡组 ,将浸泡组犬在海水中浸泡 4小时后捞出 ,然后于伤后 4、7、10、2 0小时和 2 8小时分别取血检测循环内皮细胞 (CEC)以及血管性假血友病因子 (vWF)的变化。非浸泡组除不浸泡海水外 ,余检测时相及指标同浸泡组。结果 非浸泡组在致伤后 4小时CEC和vWF明显增高 (P <0 .0 5 ) ,而浸泡组于伤后 4、7、10、2 0小时和2 8小时的CEC和vWF均明显增高 (P <0 .0 5或P <0 .0 1) ;组间比较发现浸泡组从伤后 4小时至 2 8小时各指标均与非浸泡组有显著差异 (P <0 .0 5或P <0 .0 1)。结论 烧冲复合伤合并海水浸泡伤后可引起全身EC急性损伤 ,并且这种损伤比单纯烧冲复合伤 (不浸泡 )更严重 ,更持久。

Abstract:Objective To study the effects on endothelial cell injury associated with seawater immersion after burn blast combined injury which may help to improve the early therapeutic effect.?Methods The dogs with burn blast combined injury were randomly divided into two groups: immersion group and control group. In immersion group,the dogs were immersed in seawater for 4 hours,then were taken out from seawater. Blood samples were withdrawn from central vein before wound,immediately after immersion,3h after immersion,6h after immersion,16h after immersion and 24h after immersion to measured circulating endothelial cells (CEC) and von Willebrand Factor (vWF). Parameters examined in the control group were the same as immersion group except without immersion.?Results The level of CEC and vWF elevated in control group; The level of CEC and vWF significantly elevated at every time period in immersion group which were significantly different from that found in the control group.?Conclusion Seawater immersion after burn blast combined injury would cause extensive acute damage of endothelial cells. The damage of immersion and burn blast combined injury is more severe and longer than that of simple burn blast combined injury.
