妇科疾病合并糖尿病62例围手术期的处理

探讨糖尿病患者妇科手术的处理原则。回顾性分析６２例行妇科手术的糖尿病患者的临床资料。结果全部病例术后２４小时内血糖均较术前增高。其中全麻及手术时间１２０ｍｉｎ以上对血糖的影响分析明显大于连续理解上麻醉及手术时间１２０ｍｉｎ以下。

Management of 62 gynecological patients with diabetes in the perioperative period

Objective To discuss the principles of perioperative management of gynecological surgery in diabetic patients.Methods Clinical analysis of 62 diabetic Patients with gynecologic surgery was carried out retrospectively.Results In 24 hours after opertion, all patients' blood glucose level were higher than that of properation. General anesthesia and operative time over 120 minutes affected blood glucose more obviously than continuous peridural anesthesia and opeative time less than 12O minutes. Temporary urine ketone was detected in 17 Patients, only 3 patients had consistent ketoacidosis and hypokalemia because of fasting and gastrointestinal decomprssion, but recovered by the infusion of insulin. Outcomes of all cases were good. Conclusion- Careful monitoring and active treatment must be given to the patients with diabetes prior to surgery, the method of anesthesia should be selected according to different condition, for those with posoperative fasting and gastrointestinal decompression, hypokalemia must be prevented.
