静脉滴注血必净对体外循环肺损伤的保护作用研究

摘    要:目的:研究血必净注射液对体外循环(CPB)肺损伤的保护作用。方法:随机将12只健康成年犬分为血必净注射液(治疗)和对照组,每组6只:治疗组给予血必净注射液4mL·kg-1+生理盐水100mL于切皮后至CPB前缓慢静脉滴注;对照组用等量生理盐水代替。复制犬CPB模型,分别于CPB前(T1)、开放升主动脉后30min(T2)、60min(T3)股动脉采血以放免法检测血浆肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平;同时于相应时点采集肺组织测湿/干重比,并进行病理学检查。结果:CPB后各时点TNF-α、IL-6和IL-8水平均较CPB前升高(P<0.05);对照组TNF-α、IL-6和IL-8水平在CPB后各时点均高于治疗组(P<0.05);肺湿/干重比、病理形态学改变治疗组显著优于对照组。结论:血必净注射液能减轻CPB后炎症反应,对CPB肺损伤有保护作用。

Abstract:OBJECTIVE: To discuss the protective effect of Xuebijing injection on the injury of lungs following extracorporeal circulation (CPB). METHODS: A total of 12 healthy adult dogs were randomly assigned to receive slow drip infusion of Xuebijing injection 4 mL · kg^ -1 plus NS 100 mL after incision prior to CPB(n = 6, treatment group) or same quantity of NS alone (n = 6, control group). Then the CPB model was established on the dogs. The femoral artery blood samples were taken before CPB (T1), 30 min(T2) and 60 min(T3) after the ascending aorta was open respectively for the determination of TNF- α, IL- 6, and IL-8 levels by radioimmunoassay; meanwhile, the lung tissues were taken at the same points for the measurement of the ratio of wet weight/dry weight and pathological examination was performed as well. RESULTS: The levels of TNF-α, IL-6, and IL-8 at different time points after CPB were elevated as compared with before CPB(P〈0.05); the levels of TNF-α, IL-6, and IL-8 at different time points after CPB in the control group were all higher than in the treatment group(P 〈 0.05); both the ratio of wet weight/dry weight and pathological change in the treatment group were superior to those in the control group. CONCLUSION: Xuebijing injection can reduce the inflammatory reaction and protect lungs from injury in dogs after CPB.
