先天性胆总管囊肿的诊治

目的探对先天性胆总管囊肿的诊治及疗效.方法22例先天性胆总管囊肿中,14例行胆总管囊肿切除,肝总管空肠Roux-en-Y吻合术;8例行囊肿切除术间置空肠代胆道加矩形瓣术.结果22例患者均治愈出院,无1例死亡.术后随访18例,随访时间1.5-9.5年,平均4.5年,优良率为90%.结论胆总管囊肿切除,肝管空肠Roux-en-Y吻合与间置空肠代胆道加矩形瓣抗返流术是先天性胆总管囊肿切除后胆道重建适宜选用的手术.

Diagnosis and treatment of 22 patients with congenital choledochal cyst

Objective To study the curative effect of congenital choledochal cyst.Methods Twenty-two cases of congenital choledochal cyst undergoing resection and two kinds of biliary tract reconstruction were followed up,in whom 14 cases were operated on with choledochocyst resection and Roux-Y hepaticojejunostomy,the other 8 cases were operated on with choledochocyst resection and hepaticojeunoduodenostomy plus antireflux rectangle valvoplasty.Results All patients were healed and none of them died.Eighteen cases were followed up from 1.5 years to 9.5 years(4.5 years on average).Eleven cases had excellent results,5 good,and 2 poor.Conclusion It is suggested that choledochocyst resection,Roux-Y hepaticojejunostomy,and hepaticojeunoduodenostomy are suitable for biliary tract reconstruction after resection of congenital choledochal cyst.
