2型糖尿病的中医证型与胰岛素抵抗及炎症因子的相关性研究

目的：观察2型糖尿病的中医证型与胰岛素抵抗及炎症因子的相关性。方法：将符合西医诊断标准的初发2型糖尿病患者98例,根据中医证型分为3组,将FPG、PPG、FINS、HbA1C、HOMA-IR及炎症因子（TNF-a、IL-6、IL-10）等进行比较分析。结果：中医辨证分型以气阴两虚型最多;各组间HOMA-IR、TNF-a及IL-6比较,阴虚热盛型〉气阴两虚型〉阴阳两虚型;胰岛素抵抗与炎症因子呈正相关。结论：胰岛素抵抗及炎症因子可作为2型糖尿病中医辨证分型的参考指标。

Study on relationship of traditional Chinese medicine syndrome types with insulin resistance and inflammatory factors in patients with type 2 diabetes

Objective ：To investigate the relationship of traditional Chinese medicine （TCM） syndrome types with insulin resistance and inflammatory factors in patients with type 2 diabetes. Methods：Ninety -eight patients with in cipient type 2 diabetes meeting the Western medicine diagnostic criteria were divided into three groups according to their TCM syndrome types. Fasting plasma glucose, postprandial glucose, hemoglobin A1 c, homeostasis model assess- ment - estimated insulin resistance （ HOMA - IR）, and inflammatory factors （ tumor necrosis factor alpha （ TNF - α）, interleukin- 6 （IL- 6） ,and interleukin- 10） were compared between the two groups. Results：Deficiency of both qi and yin was the most common TCM syndrome type. Patients with syndrome of yin deficiency and heat excess had high- er HOMA - IR and levels of TNF -α and IL - 6 than those with syndrome of qi - yin deficiency, whose HOMA - IR and levels of TNF - ot and IL - 6 were higher than those of patients with syndrome of yin - yang deficiency. Insulin re- sistanee was positively correlated with inflammatory factor levels. Conclusion：Insulin resistance and inflammatory fac- tors can be used as reference indices in TCM syndrome differentiation among patients with type 2 diabetes.
