摘    要:目的 观察慢性肾脏病(CKD)患者血清FGF-23及Klotho水平与肾功能进展的关系.方法 选取本院门诊30例健康体检者(对照组)及90例CKD1~5期的患者(CKD组),其中CKD组根据肾功能下降情况分为肾功能恶化组及肾功能稳定组;采用EHSA法测定血清FGF-23及Klotho水平,分析FGF-23及Klotho水平变化与肾功能进展的关系.结果 CKD组血清FGF-23明显高于对照组,血清Klotho明显低于对照组,且随着肾功能进展,这种变化越明显;肾功能恶化组FGF-23明显高于肾功能稳定组,而血清Klotho明显低于肾功能稳定组;相关分析显示,FGF-23与Klotho之间呈负相关.结论 CKD患者血清FGF-23水平明显升高,血清KLotho明显降低,与肾功能进展有关,可作为CKD患者肾功能恶化及不良预后的指标.

Serum levels of fibroblast growth factor-23 and Klotho and their relation to kidney function deterioration in chronic kidney disease patients
Abstract:Objectives To analyze the relation of serum fibroblast growth factor-23 (FGF-23) and Klotho and their relation to the deterioration of kidney function in patients with chronic kidney disease (CKD).Methods 90 patients with CKD1 to CKD5 stages (CKD group) and 30 non-CKD patients (control group) were enrolled.Patients in CKD group were divided into deterioration of renal function group and stability of renal function group based on the annual decline rate of eGFR.The serum levels of FGF-23 and Klotho among all groups were determined by enzyme-linked immunosorbent assay (ELISA).The correlation of FGF-23 and Klotho and aggravation of renal function were analyzed.Results The level of FGF-23 in CKD group was significantly higher,while the level of Klotho was lower than that of control group.The characteristic changes in FGF-23 and Klotho were also detected as renal function progresses.The similar results were observed between groups of renal function deterioration and renal function stability.Pearson correlation analysis showed that serum FGF -23 in CKD patients was negatively correlated with Klotho.Conclusions FGF-23 levels in CKD patients would increase and Klotho levels would decrease when kidney function deteriorates.Serum FGF-23 and Klotho may be related to kidney dysfunction and can be used to predict CKD progression and poor prognosis in CKD patients.
