“U”字形法经尿道双极等离子前列腺剜除术体会

目的 探讨采用经尿道双极等离子“U”字形前列腺腔内剜除术方法体会.方法 本院自2013年5月至2015年9月,使用双极等离子电切系统采用“U”字形前列腺腔内剜除术治疗100例前列腺增生患者,通过分析患者术前、术后尿路指标、手术时间、术中出血、导尿管留置时间等相关指标,评估经尿道双极等离子“U”字形前列腺剜除术的安全性、有效性.结果 100例患者均成功采用经尿道双极等离子“U”字形法完成前列腺腔内剜除手术.无死亡、输血和电切综合征发生.术后最大尿流率明显大于术前(t =15.912;P=0.000);术后残余尿明显少于术前(t =44.069,P=0.000),术后随访无逆行射精、性功能障碍和排尿困难等并发症发生.结论 “U”字法经尿道双极等离子前列腺剜除术治疗前列腺增生症具有安全、有效、设备条件要求低等优点.

Experience of bipolar plasmakinetic transurethral enucleation of prostate with "U" technique

Objectives To investigate experience of bipolar plasmakinetic transurethral enucleation of prostate with "U" technique.Methods 100 patients with benign prostatic hyperplasia (BPH) were treated by bipolar plasmakinetic transurethral enucleation of prostate with "U" technique from May 2013 to September 2015.The urinary tract index,operative time,blood loss,indwelling catheter time before and after surgery were analysed.The safety and effectiveness of bipolar plasmakinetic transurethral enucleation of prostate with "U" technique were evaluated.Results All patients were treated successfully,there was no death,blood transfusion and transurethral resection syndrome happened.Postoperative maximum flow rate was significantly higher than before surgery (t =15.912,P =0.000),postoperative residual urine was significantly less than before surgery (t =44.069,P =0.000),and there was no retrograde ejaculation,sexual dysfunction and dysuria occurred in the postoperative follow-up.Conclusions Bipolar plasmakinetic transurethral enucleation of prostate with "U"technique has advantages in safety,efficacy and low equipment condition and so on.
