取石网篮在输尿管镜下钬激光碎石治疗上段输尿管结石中的应用

目的 探讨取石网篮在输尿管镜下钬激光碎石治疗上段输尿管结石中的临床应用.方法 将上段输尿管结石80例随机分为观察组和对照组各40例,观察组采用NGage取石网篮配合输尿管镜下钬激光碎石,对照组采用单纯的输尿管镜下钬激光碎石.结果 观察组患者碎石成功率为95％,显著高于对照组的75％,差异有统计学意义(P＜0.05),观察组的结石残留率为5％,明显低于对照组的20％,差异同样有统计学意义(P＜0.05),而两组在并发症发生率、手术时间以及住院时间上差异无统计学意义(P＞0.05).结论 取石篮网配合输尿管镜钬激光碎石治疗上段输尿管结石,可显著提高一次性结石成功率,降低结石的残留率,具有良好的有效性和安全性.

Extraction basket in ureteroscope holmium laser lithotripsy for upper ureteral calculi

Objectives To investigate the clinical application value of ureteroscope holmium laser lithotripsy with NGage extraction basket in the treatment of upper ureteral calculi.Methods Totally 80 patients with upper ureteral calculi were randomly divided into two groups,40 patients in control group were treated by ureteroscope holmium laser lithotripsy,40 patients in observation group were treated by ureteroscope holmium laser lithotripsy combined with NGage extraction basket.Results The success rate of primary lithotripsy in observation group were 95％,which was higher than that in control group(75％),and the difference was statistically significant(P ＜ 0.05).The rate of residual calculi in observation group were 5％,which was lower than that in control group (20％),and the difference was statistically significant (P ＜ 0.05).There was no significant difference between two groups in the operation time,complication rate and hospitalization time.Conclusions NGage extraction basket with ureteroscope holmium laser lithotripsy could efficaciously prevent calculi from moving upward in the treatment of upper ureteral calculi,which could achieve higher success rate and lower residual calculi rate.
