布托啡诺预防应用卡列前素氨丁三醇治疗产后出血所致不良反应的临床效果

目的观察布托啡诺预防应用卡列前素氨丁三醇治疗产后出血所致不良反应的临床效果。方法选取2018年4月-2019年4月福建医科大学附属三明第一医院收治的使用卡前列素氨丁三醇治疗产后出血患者70例进行研究,采用随机分组表法将患者分为干预组(35例)和常规组(35例)。2组均给予卡前列素氨丁三醇治疗产后出血,干预组同时给予布托啡诺静脉泵注。比较2组患者治疗前及治疗1、7 d睡眠质量评分,治疗前、治疗7 d不良情绪评分及相关不良反应发生率。结果治疗1、7 d,2组睡眠质量评分较治疗前升高(P<0.01),但干预组睡眠质量评分均低于常规组(P<0.01)。治疗前,2组HAD-A、HAD-D评分比较无显著差异(P>0.05);治疗7 d,干预组患者HAD-A、HAD-D评分均显著低于常规组(P<0.01)。2组患者治疗期间恶心呕吐、血压升高及腹泻发生率比较差异均无统计学意义(P>0.05)。结论布托啡诺预防应用卡前列素氨丁三醇治疗产后出血患者所致不良反应的效果确切,可降低患者不良情绪、睡眠障碍程度,且其他相关并发症发生率低。

Clinical effect of butorphanol in the prevention of adverse reactions of postpartum hemorrhage

Objective To observe the clinical effect of butorphanol in the prevention of adverse reactions caused by the application of prednisolone tromethamine in the treatment of postpartum hemorrhage.Methods 70 patients with postpartum hemorrhage treated with carboprost tromethamine in Sanming First Hospital Affiliated to Fujian Medical University from April 2018 to April 2019 were selected and randomly divided into intervention group(35 cases)and conventional group(35 cases).Both groups were treated with carboprost tromethamine for postpartum hemorrhage,while the intervention group was given butorphanol by intravenous pump.The sleep quality scores before treatment,1 day and 7 days of treatment were compared between the two groups.The adverse emotion scores and incidence of related adverse reactions were compared between the two groups before treatment and 7 days after treatment.Results After 1 day and 7 days of treatment,the sleep quality scores of the two groups were higher than those before treatment(P<0.01),but the sleep quality scores of the intervention group were lower than those of the conventional group(P<0.01).Before treatment,there was no significant difference in the HAD-A、HAD-D scores between the two groups(P>0.05).After 7 days of treatment,the HAD-A、HAD-D scores of the intervention group were significantly lower than those of the conventional group(P<0.01).There was no significant difference in the incidence of nausea and vomiting,increased blood pressure and diarrhea between the two groups(P>0.05).Conclusion Butorphanol can effectively prevent adverse reactions caused by carboprost tromethamine in the treatment of postpartum hemorrhage.It can reduce the degree of adverse emotions and sleep disorders,and the incidence of other related complications is low.
