局部麻醉关节镜下内引流技术治疗腘窝囊肿的疗效观察

目的:探讨局部麻醉关节镜内引流技术治疗腘窝囊肿的手术方法和临床疗效。方法:采用关节镜技术后内侧入路治疗腘窝囊肿48例,局部麻醉,术中常规处理膝关节腔内病变后,探查半膜肌-腓肠肌内侧头间横行瓣口结构并扩大通道。术后功能恢复采用Rauschning-Lindgren分级标准,疼痛模拟评分(VAS)评价临床效果。结果:术后Rauschning-Lindgren分级明显优于术前(P<0.05);术后3个月VAS评分优于术前(P<0.05),相对于术前症状均有显著改善;无切口感染及后方大血管神经损伤;术后随访6~18个月,囊肿均消失,无复发。结论:局部麻醉关节镜下内引流技术治疗腘窝囊肿是一种微创、安全有效、复发率低、术后康复快的方法。

Treatment of popliteal cyst by arthroscopic internal drainage under local anesthesia

Objective:To explore the surgical method and clinical effect of local anesthesia arthroscopic drainage technique in the treatment of popliteal cyst.Methods:All 48 cases were treated by the medial approach of arthroscopy and local anesthesia.After the conventional treatment of the intra-articular lesions of the knee joint,the structure of the transverse flap between the medial and gastrocnemius muscle of the semi-membrane was explored and the channel was enlarged.The postoperative functional recovery was evaluated using the Rauschning-lndgren classification standard,and the pain visual analogue scale(VAS)score was used to evaluate the post-operative clinical effect.Results:The grade of Rauschning and Lindgren after operation was significantly better than that before operation(P<0.05).The VAS score at 3 months after operation was better than that before operation(P<0.05),and the symptoms were significantly improved compared with those before operation.There was no incision infection and posterior macrovascular nerve injury,and the cysts disappeared and no recurrence occurred after 6 min 18 months follow-up.Conclusion:Arthroscopic internal drainage under local anesthesia is a minimally invasive,safe and effective method for the treatment of popliteal cysts with low recurrence rate and rapid postoperative recovery.
