人体耻骨上、下支显微骨硬度分布特征研究

摘    要:目的 探讨人体耻骨上、下支显微硬度的分布特征。方法 由河北医科大学解剖教研室提供3具新鲜冰冻成人尸体标本(标本A为62岁男性、B为45岁女性、C为58岁男性),取出耻骨支并剔除附着之软组织。使用微型台锯及高精慢速锯将耻骨上、下支各分成3等份,每份进行均分切割,将骨骼制成厚3 mm的骨组织切片,固定在载玻片上并进行标记。依次用800、1 000、1 200、2 000、4 000目碳化硅粒砂纸打磨标本。应用德国KB-5型显微维氏硬度仪测试标本不同区域的硬度值,采用50 gf力加载50 s、维持12 s的标准操作方法进行硬度值测定,每一个区域选取5个有效显微硬度值,全体有效值的平均值为该区域的硬度值。结果 A、B、C 3具标本耻骨上支骨硬度值分别为(27.99±6.03)HV、(29.93±4.86)HV、(33.42±5.15)HV, 耻骨下支骨硬度值分别为(33.99±3.10)HV、(37.95±5.39)HV、(36.19±3.87)HV,3组数据比较差异均有统计学意义(F上支=9.540、F下支=12.890, P值均<0.01)。3具标本耻骨下支骨硬度值均高于上支,差异均有统计学意义(tA=3.432、tB=4.280、tC=3.874, P值均<0.01)。结论 人体耻骨下支显微骨硬度高于耻骨上支。这一耻骨显微骨硬度分布特征的首次发现,可辅助阐释耻骨支骨折的损伤机制,为研发符合该部位骨硬度特点的内固定物,及相关生物力学研究提供数据支持。临床试验注册 中国临床试验注册中心,注册号为ChiCTR-TNC-17010818。

Abstract:Objective To investigate the distribution characteristics of micro-vickers hardness (HV) in the pubic ramus.Methods Three fresh frozen adult cadavers were provided by the Department of Anatomy of Hebei Medical University. Specimen A, B, C were a 62-year-old male, a 45-year-old female and a 58-year-old male, respectively. Pubic ramus were taken out and soft tissue was removed. Micro-table saw and high-precision slow-speed saw were used to cut the specimens. Each one of the superior and inferior ramus of pubic were divided into 3 equal parts,each of which was equally cut. The bone was made into bone tissue sections 3 mm thick, which were fixed on slides and marked. The samples were sanded with silica carbide particles of 800, 1 000, 1 200, 2 000 and 4 000 mesh respectively. The hardness values of pubic ramus in different regions were tested by German KB-5 microhardness tester. In this series of studies, the standard operating method of 50 gf force loading 50 s and maintaining 12 s was used to determine the hardness values. Five effective microhardness values were selected for each region, and the average value of all effective values was used to be the hardness value of the region.Results The average bone hardness of superior and inferior ramus of pubic were (27.99±6.03)HV, (29.93±4.86) HV, (33.42±5.15)HV, and (33.99±3.10) HV, (37.95±5.39) HV, (36.19±3.87) HV, respectively. The One-way Anova analysis showed the significant difference of superior and inferior ramus of pubic among 3 groups(Fsr=9.540, Fir=12.890, all P values<0.01). The bone hardness of the inferior ramus of the pubis was higher than that of the superior ramus, and the difference was statistically significant (tA=3.432, tB=4.280, tC=3.874, all P values<0.01) for the three groups.Conclusions The microhardness of the inferior ramus of the pubis is higher than that of the superior ramus of the pubis. This is the first discovery of the distribution characteristics of microscopic bone hardness of the pubic bone,which can help to explain the injury mechanism of the pubic branch fracture, and provide data support for the development of internal fixation that conforms to the characteristics of bone hardness of the site and related biomechanical research.
