纤维支气管镜肺灌洗联合无创正压通气治疗重症肺部感染疗效观察

摘    要:目的探讨纤维支气管镜肺灌洗联合无创正压通气NIPPV对重症肺部感染的治疗效果。方法对14例重症肺部感染患者在综合治疗的基础上行NIPPV和纤维支气管镜肺灌洗。监测患者生命体征、动脉血气分析,对治疗前、后各项监测指标及临床情况进行对比分析。结果大部分患者治疗后取得良好效果,pH值正常、PaO2上升、PaCO2下降,ACS评分降低,外周血白细胞下降至正常,胸片明显改善。结论纤维支气管镜肺灌洗联合NIPPV治疗重症肺部感染是安全可行的,临床疗效肯定。

Abstract:Objective To explore the clinical effect of bronchoscopy bronchoalveolar lavage and non-invasive positive pressure ventilation (NIPPV) in the treatment of severe pulmonary infection. Methods 14 patients with severe pulmonary infection were given bronchoalveolar lavage and NIPPV on the basis of the comprehensive treatment. The physical signs and blood gases were analyzed and compared before and after the treatment. Results Most patients gained good effect after the treatment with normal pH, increased PaO2 , decreased PaCO2 , reduced ACS scores, normal WBC level and significant improvement of chest X-ray. Conclusion Bronchoalveolar lavage combined with NIPPV is a safe and effective method in the treatment of severe pulmonary infection and it's worth to be applied widely in clinical application.
