急性胆囊炎的治疗体会

目的：探讨关于急性胆囊炎的合理治疗方法：方法：回顾了234例急性胆囊炎患者的治疗过程及结果。结果：190例（81％）患者经保守治疗后好转，44例（19％）患者保守治疗中转手术，其中胆囊切除者21例（47．7％），胆囊部分切除粘膜剔除者6例（13．6％），胆囊造瘘者15例（34．1％），术后切口感染2例，胆总管损伤1例（胆总管缝扎），死亡1例（死于术后急性心梗），结论：应严格掌握急性胆囊炎的手术时机，根据病人的情况选择恰当的治疗方案。

The experience of treatment in acute cholecystitis

Objective: To study the appropriate treatments in acute cholecystitis. Methods:Reviewed the management processes and results of 234 patients with acute cholecystitis.Results:190(81%) patients were cured with nonoperative treatments,the other 44(19%) patients were received operation,when nonoperative treatment were failure.Among them,cholecystectory were performed in 21(47.7%) patients,partial cholecystectomy in 6(13.6%)patients,cholecystotomy in l5(34.1%) patients. Postoperation 1 with wound infection,1 with common bile duct injury(common duct ligation),1 with bile leak,l with postoperative choledocholithiasis,1 died in acute postoperative myocardial infarction.Conclusion:We should control the operative indication of acute cholecystitis strictly and choose the appropriate treatment according to the condition of patients.
