伊曲康唑注射液治疗老年人侵袭性肺曲霉病

目的:研究伊曲康唑注射液治疗老年人侵袭性肺曲霉病的临床疗效及安全性。方法:对15例60 a以上应用伊曲康唑治疗的侵袭性肺曲霉病病人的资料进行分析。其中男性11例,女性4例,年龄(72±s 7)a;确诊病例3例,临床诊断很可能病例8例,临床诊断可能病例4例。伊曲康唑注射液d 1～2,每日400mg,分2次静脉滴注,以后200 mg·d-1静脉滴注。观察临床疗效、用药时间、预后及不良反应。结果:总有效率60％(9／15)。3例确诊病例中显效1例,进步1例,无效1例。临床诊断很可能病例中痊愈2例,显效2例,无效4例。临床诊断可能病例中痊愈3例,显效1例。曲霉菌完全清除9例,真菌交替3例,未清除3例。不良反应为头痛1例。结论:早期、足量、足疗程应用伊曲康唑注射液治疗老年人侵袭性肺曲菌病疗效好,安全。

Itraconazole injection in treatment of invasive pulmonary aspergillosis in elders

AIM: To evaluate the efficacy and safety of itraconazole injection in the treatment of invasive pulmonary aspergillosis in the elders. METHODS; The clinical data of 15 elder patients (M 11, F 4, age (72±s 7) a) with invasive pulmonary aspergillosis admitted to department of respiratory disease in our hospital from January 2004 to March 2006 were reviewed retrospectively. Definite invasive pulmonary aspergillosis included 3 patients, probable 8 patients, possible 4 patients. The patients were given itraconazole injection by intravenous infusion, d 1-2, 200 mg, bid; hereafter 200 mg·d-1; simultaneously with observation of clinical efficacy, safety, drug administration time and adverse reactions. RESULTS: The total clinical efficacy rate was 60 % (9/15) . The clinical efficacy rates were 33 % (1/3) among definite invasive pulmonary aspergillosis, 50 % (4/8) among probable invasive pulmonary aspergillosis, 100 % (4/4) among possible invasive pulmonary aspergillosis. The fungal eradication was obtained in 9 patients, fungal alternative in 3 patients and no eradication in 3 patients. Headache occurred in one patient. CONCLUSION: Itraconazole injection is effective and well tolerated in the treatment of invasive pulmonary aspergillosis in elders with early application at the onset and adequate dosage for a sufficiently long period.
