超声监测筛查胎儿先天性心脏畸形

目的探讨超声筛查胎儿先天性心脏畸形的临床价值。方法对常规产前超声检查在胎儿四腔心中疑有异常,临床发现胎儿心律不齐,以及母体有高危因素者共12例重点做超声心动图检查。结果超声检查发现12例先天性心脏异常中,10例为先天性心脏病,其中单纯性室缺2例,右位心、内脏转位1例,复杂性先天性心脏病7例;2例为心脏肿瘤,其中室间隔横纹肌瘤1例,多发性黏液瘤1例。12例心脏异常患者中2例同时合并其他器官结构异常,3例脐动脉A/B值异常,4例心律紊乱。结论产前超声筛查胎儿先天性心脏畸形具有重要的临床价值。对有高危因素,胎儿心律失常及其他内脏畸形者,应重点行超声心动图检查。

Screening the fetal congenital heart disease by ultrasound monitoring

Objective To investigate the clinical value in screening fetal congenital heart disease(CHD) with ultrasound.Methods Authors examined 12 foetus by echocardiogram. For all the 12 cases, some pregnant women had high risk factors and some foetus were arrhythmia and other foetus were doubted abnormal on ultrasonography image of the four-chamber view(4CV) through prenatal regular examination.Results Of 12 cases, 10 cases were CHD and 2 cases were tumor of heart. In the 10 CHD cases, 2 cases were VSD, 1 case was dextrocardia combining heterotaxia and 7 cases were complex CHD. Two heart tumor cases were ventricular rhadomyosarcoma and multiple myoxomyoma respectively. In the 12 cases, 2 cases suffered from abnormal structure in other organs, 1 case was dextrocardia combining heterotaxia,4 cases were arrhythmia and umbilical artery A/B were abnormal.Conclusions Prenatal ultrasonic examination possess important clinical value in screening fetal CHD. If pregnant women have high risk factors or the foetus suffer from arrhythmia and other fetal malformation, echocardiogram examination on foetus especiall should be made.
