摘    要:目的:探讨双源CT头颈联合血管成像在椎-基底动脉供血不足性眩晕患者中对椎-基底动脉病变及变异的诊断价值。方法:收集我院2011至2012年期间的833例因椎-基底动脉供血不足性眩晕患者,使用双源CT行头颈联合血管成像检查,所得数据行容积重建,最大密度投影、多平面重建、曲面重建、狭窄程度分析等后处理,判断椎基底动脉情况。结果:833例患者中706例患者存在椎-基底动脉病变及变异,其中最多见为动脉粥样硬化及发育不良。结论:双源CT头颈联合血管成像可清楚地显示椎-基底动脉全貌,椎-基底动脉主干及其分支、侧枝循环均显示良好,其对于椎-基底动脉疾病及变异的诊断具有重要意义,为微创、简便、安全、有效的检查手段。

Abstract:Objective:To explore the diagnostic value of dual-source computed tomography head-and-neck angiography in the lesion and variation of vertebral-basilar artery for patients with vertebral-basilar insufficiency ( VBI) circumgyration. Methods:833 in-patients with circumgyration symptoms during 2011-2012 accepted the examination of dual-source computed tomography angiography. The image data of vertebrobasilar artery were post processed by volume rendering (VR),maximum intensity projection (MIP),multiple planar reformation ( MPR) ,curved planar reformation ( CPR) and advanced vessel analysis ( AVA) to display the variation of verte-brobasilar artery:origin,path variation and branches. Results:706 cases were diagnosed lesion and variation of vertebral-basilar artery. The most common cases of lesions were atherosclerosis and dysplasia of carotid artery. Conclusion:Dual-source computed tomography angiography can clearly show the full view of vertebral-basilar artery. The origin, path variation and branches of artery can be well shown. It contains great diagnostic value of lesion and variation of vertebral-basilar artery and can be selected as a minimal invasive, simple,safe and effective way of inspection.
