胰岛素泵治疗妊娠糖尿病的临床疗效分析

目的探讨胰岛素泵治疗妊娠糖尿病的临床疗效及安全性。方法选择2008年2月至2011年2月160例妊娠糖尿病围术期患者,随机分入对照组与观察组,每组各80例。对照组多次皮下胰岛素注射治疗,观察组胰岛素泵持续皮下输注,比较两组干预前后血糖水平、血糖达标时间、低血糖及母婴并发症发生率。结果两组治疗后血糖水平均明显下降（P〈0.01）,但观察组血糖优于对照组（P〈0.05）;观察组血糖达标时间少于对照组（P〈0.05）;观察组低血糖发生率低于对照组（5.0%〈12.5%,P〈0.05）,观察组母婴并发症发生率少于对照组,差异具有统计学意义（P〈0.05）。结论妊娠糖尿病患者使用胰岛素泵降糖达标时间快,血糖平稳,且安全性高。

Clinical effect and safety analysis on insulin pump treatment in gestational diabetes mellitus

Objective To explore the clinical effect and safety of insulin pump treatment in gestational diabetes mellitus.Methods 160 cases with gestational diabetes mellitus were randomly divided into control group and observation group,80 patients for each group.Observation group was given continuous subcutaneous insulin injection by insulin pump and control group received multiple subcutaneous insulin injection.Blood glucose level,time of reaching target of glucose control,incidence of hypoglycemia and maternal and neonatal complications were compared between two groups.Results Blood glucose level greatly decreased after treatment in two groups（P0.01）,but level of blood glucose control in observation group was superior to control group（P0.05）.Time of reaching target of glucose control in observation group was less than that in control group（P0.05）.Incidence of hypoglycemia in observation group was much lower than that in control group（5.0% vs 12.5%,P0.05）.Incidence of maternal and neonatal complications in observation group was much lower than that in control group（P0.05）.Conclusion Insulin pump treatment for gestational diabetes mellitus could acquire optimal glucose level rapidly and stably with high safety.
