大肠埃希菌和克雷伯菌属超广谱β-内酰胺酶的检测

摘    要:目的通过对临床分离的大肠埃希菌、克雷伯菌属及其它肠杆菌科超广谱β-内酰胺酶(ESBLs)的检测,探讨该酶介导的耐药性在临床应用中的作用。方法收集临床分离的耐氨苄西林的大肠埃希菌和克雷伯菌属和其它肠杆菌细菌共59株,按NCCLs推荐的初筛、表型确证试验、Etest法操作。结果有21株细菌被确认为产ESBLs,占收集菌株的35.6%,其中确认试验与Etest均阳性的有13株;产ESBLs的大肠埃希菌的检出率为32.4%,克雷伯菌属为50.0%,其它肠杆菌科细菌为30.8%。结论肺炎克雷伯菌和大肠埃希菌是产生超广谱β-内酰胺酶的主要菌株,但是其他肠杆菌科细菌的检出率有所上升,尤其是不能忽视阴沟肠杆菌的检测。本地区用于超广谱β-内酰胺酶筛选的最佳底物是头孢噻肟(CTX)。

Abstract:Objective To evaluate the effect ofβ-lactamases on the clinic by examining klebsiella species,E.coli and other enteric bacilli.Methods Fifty-nine strains of ampicillin-resistant klebsiella species and Escherichia Coli were collected.The suspiciously produced ESBLs was detected by standard screen test among the59E.coli and K.pneumoniae.Strains possessing ESBLs were confirmed by E test.Results Twenty-one isolates gave positive results by confirmative test(35.6%).The detected rate of E.coli,K.pneumoniae and other enteric bacilli bacteria was32.4%,50.0%and30.8%,respectively.Conclusion K.pneumoniae and E.coli are main isolates producing ESBLs.The best antibiotic screening ESBLs is cefotaxime(CTX).
