超声造影鉴别睾丸占位性病变良恶性的价值

目的：探讨超声造影（contrast-enhanced ultrasound，CEUS)在睾丸占位性病变良恶性鉴别诊断中的价值。 方法：回顾性分析我院术前经CEUS检查并术后经病理证实的81例睾丸占位性病变的CEUS特征，通过比较不同性质睾丸占位性病变的CEUS特征来鉴别其良恶性，并采用多因素二元Logistic回归分析各项CEUS指标来预测睾丸占位性病变的恶性风险程度。 结果：睾丸占位性病变的增强边界、始增速度、增强后病灶大小差异均有统计学意义（P<0.05），增强均匀程度、增强水平、廓清速度差异无统计学意义（P>0.05）。二元Logistic回归分析筛选出3个与睾丸占位性病变恶性相关的CEUS特征，依次是增强边界不清、快进、增强后病灶范围增大。 结论： CEUS对鉴别睾丸占位性病变的良恶性具有重要价值。

Value of contrast-enhanced ultrasound in differentiating benign from malignant testicular space-occupying lesions

Objective To explore the value of contrast-enhanced ultrasound (CEUS) in the differential diagnosis of benign and malignant testicular space-occupying lesions. Methods:The CEUS characteristics of 81 testicular space-occupying lesions examined by CEUS before operation and confirmed by pathologically in our hospital were retrospectively analyzed. The CEUS characteristics of testicular space-occupying lesions were compared to identify benign and malignant, and the multivariate Binary logistic regression analysis of various CEUS indicators were used to predict the degree of malignant risk of testicular space-occupying lesions.Results: There were significant differences in the boundary of enhancement,the speed of contrast agent into testicular space-occupying lesions and enhanced lesion size of testicular space-occupying lesions(P<0.05),There was no significant difference in the degree of enhancement uniformity, enhancement level and clearance rate (P>0.05).three CEUS characteristics associated with malignancy of testicular space-occupying lesions were obtained by binary logistic regression analysis, followed by unclear enhancement boundary, fast wash in, and increased lesion size after enhancement.Conclusions:CEUS provides important value in differentiating benign from malignant testicular space-occupying lesions.
