TI-RADS联合剪切波弹性成像对甲状腺良恶性结节的鉴别诊断价值

摘    要:目的:为了探究剪切波超声弹性成像(SWE, Shear Wave Elastography)联合甲状腺成像报告和数据系统(ACR-TIRADS)鉴别甲状腺结节良性与恶性的有效性。方法:本研究共纳入例上海中医药大学附属曙光医院甲状腺结节患者129例(甲状腺结节142个),以病理学结果为金标准,使用TIRADS标准对每一个结节进行风险评分,通过ROC 曲线评估SWE参数的有效性,采用曲线下面积(AUC)比较单独采用TIRADS与联合TIRADS+SWE的诊断性能。结果:单独使用TIRADS和TIRADS+SWE的AUC值分别为0.885(准确度83.5%)和0.914(准确度83.9%)。然而,与TIRADS相比,TIRADS+SWE评分具有更高的特异性(75.9 %和89.4%)和阳性预测值(79.3 % vs 82.7%)。结论:联合SWE和TIRADS可提高TIRADS在区分良性和恶性甲状腺结节方面的特异性和准确性。

Abstract:Objective: To explore the efficacy of shear wave elastography (SWE) combined with thyroid imaging reporting and data system (ACR-TIRADS) in differentiating benign and malignant thyroid nodules. METHODS: A total of 129 patients with thyroid nodules (142 thyroid nodules) in ShuGuang Hospital were enrolled in this study. With the pathological results as the gold standard, each nodule was evaluated by TIRADS. The validity of SWE parameters was evaluated by ROCs. Using area under ROC curve (AUC) to compare the diagnostic performance of TIRADS alone and combined TIRADS with SWE. RESULTS: The AUC values of TIRADS and TIRADS+SWE alone were 0.885 (Accuracy 83.5% and 0.914 (Accuracy 83.9 %). However, compared with TIRADS, TIRADS + SWE score has higher specificity (75.9 % vs 89.4%) and positive predictive value (79.3 % vs 82.7%), respectively. Conclusion: Combination of SWE and TIRADS can improve the specificity of TIRADS in differentiating benign and malignant thyroid nodules.
