国际血液分析仪41条复检规则的临床应用评价及实验室复检标准的建立

目的采用Sysmex XS-1000i血细胞分析仪评估国际血液学专家组推荐的血细胞41条复检规则,提出适合于临床使用的血细胞复检规则。方法采用日本希森美康公司生产的XS-1000i血细胞分析仪,检测太仓市璜泾人民医院门诊和住院标本6 685份,按照国际血细胞复检规则和涂片镜检阳性标准进行评估,计算真阳性率、假阳性率、假阴性率、评价41条复检规则的临床应用价值,并筛选出最适合临床使用的血液复检规则。结果根据国际血液学复检专家组推荐的41条复检规则和涂片镜检阳性标准对检测结果进行统计学分析,复检率为38.6%(2 584/6 685),真阳性率64.20%,假阳性35.30%,假阴性率1.47%。采用筛选后的复检规则进行检测,1 021份标本复检率为14.4%,真阳性率79.96%,假阳性率20.04%,假阴性率3.23%。结论使用国际41条复检标准,漏检率低但复检率过高,不适合临床应用,采用筛选后的复检标准复检率大幅降低为14.40%,假阴性率仅为3.23%,符合国际血液学专家组确定的5%可接受限。
