肥胖患者运动血压与血糖、血浆胰岛素水平的关系

摘    要:目的 探讨肥胖患者运动血压与血糖、血浆胰岛素水平的关系。方法 选取完成次极量踏车运动试验的 49例肥胖患者 (Obesity)及 45例体重正常的对照组 (Control) ,比较其静态血压 (RBP)、运动血压 (PBP)及空腹和口服 75克葡萄糖 2小时后血糖、血浆胰岛素水平。并分析肥胖组中合并运动性高血压 (PBP1)与运动血压正常者 (PBP2 )的血糖、胰岛素水平。结果 静态下两组血压无显著差异 ,负荷试验后达到运动性高血压标准者 ,肥胖组 2 1/ 49例 (42 8% ) ,对照组 8/ 45例 (17 8% ) ,P <0 0 1;运动后肥胖组的收缩压与舒张压均较对照组升高 ,特别是收缩压升高更明显 ;两组空腹血糖无显著差异 ,肥胖组的空腹胰岛素、餐后 2小时血糖及胰岛素均明显高于对照组 ;肥胖组中伴运动性高血压者的餐后 2小时血糖、胰岛素水平均高于不伴运动性高血压的肥胖患者。结论 肥胖患者血压、血糖升高可能与胰岛素水平升高有关

Abstract:Objective To study the relationship between exercise induce d blood pressure elevation versus plasma glucose and insulin in obese patients. Methods Forty-nine ob ese patients and 45 normal controls were recruited. All subjects received glucose loading test and submaximal workload test by bicycle ergometer. Resting blood pressure(RBP), exercise blood pressure(EBP1), plasma glucose and insulin (in fasting state and 2 hours after 75 g glucose-stimulated) were measured . In obese patients with (EBP1) and without exercise hypertension (EBP2) the plasma glucose and insulin were compared. Results The resting blood pressure were not different significantly between obese patients and norma l subjects. 21/49 (42 86%) in the obese group and 8/45(17 78%) in the control group met the diagnostic criter ion of exercise hypertension after the loading test(P<0 01). Systolic and diastolic blood pressure in the obese group were higher than in the controls after exercising. While plasma fasting glucose between 2 groups were not different significantly, fasting plasma insulin and glucose-loading induced increases in plasma glucose and insulin were significantly greater in the obese group than that in the controls. Furthermore glucose-loading induced plasma glucose and insulin we re also higher in the obesity with exercise hypertension than that in obese patients without exercise hypertension. Conclusion Exereise induced hypertension is more frequently found in obe se patients and was associated with increases in fast and glucose loading insulin. Obese patients with exercise hypertension were characterstic of more pronounced metabolic disorder.
