腹腔镜胆囊切除术不同气腹持续时间对术后肩痛的影响

目的:探讨腹腔镜胆囊手术不同气腹持续时间对术后肩痛的影响。方法:于全身麻醉下为91例患者行腹腔镜胆囊切除术,气腹压力为12mm Hg,根据气腹时间分为3组,A组<1h;B组1~2h;C组>2h。记录术后清醒、拔管时间及术后3d内的肩痛情况,并让患者于术后12h、24h、48h、72h肩痛用视觉模拟评分法(visual analogue scale system,VAS)评分。结果:术后出现肩痛A组12例,B组10例,C组8例,差异无统计学意义(P>0.05)。3组患者肩痛VAS评分差异无统计学意义。结论:不同气腹时间对术后肩痛的发生率和疼痛程度无影响。

Effect of different duration of carbon dioxide pneumoperitoneum on postoperative shoulder pain occurrence following laparoscopic cholecystectomy

Objective:To explore the effect of different duration of the carbon dioxide(CO2)pneumoperitoneum in laparoscopic cholecystectomy(LC)on the occurrence of postoperative shoulder pain.Methods:Ninety-one cases who underwent LC with 12mm Hg of CO2 pneumoperitoneum after general anaesthesia,were divided into three groups according to different duration of CO2 pneumoperitoneum:group A<1 hour;group B 1-2 hour;group C>2 hour.Mean shoulder pain scores was recorded on a visual analogue pain scale 12h,24h,48h,72h after operation.Results:Postoperative shoulder pain rate and the VAS score in three groups had no significant differences(P>0.05).Conclusions:Postoperative shoulder pain rate and level has no significant differences with the duration of CO2 pneumoperitoneum in laparoscopic cholecystectomy.
