便携式超声雾化器防治鼻咽癌放疗后耳鼻咽喉并发症的临床价值

摘    要:目的探讨鼻咽癌放疗后应用便携式超声雾化器雾化吸入治疗的方法和临床疗效。方法随机选择2010年3月至2013年3月放疗后鼻咽癌患者60例,随机分为实验组和对照组各30例,分别采用超声雾化吸入疗法和普通生理盐水洗鼻,每天早晚各1~2次,疗程3个月,比较两组患者鼻黏膜反应及并发症发生情况。结果实验组患者鼻咽黏膜轻度反应(0~Ⅰ度)比例为40%,重度反应(Ⅱ~Ⅳ度)为60%,对照组轻度反应占10%,重度反应占90%,实验组患者鼻黏膜反应明显轻于对照组,差异具有统计学意义(P〈0.05);实验组患者咽喉干燥感、渗出性中耳炎、鼻-鼻窦炎、鼻腔粘连、后鼻孔狭窄等并发症发生率分别为23.3%、26.7%、6.7%、3.3%、3.3%,均明显低于对照组的76.7%、80.0%、53.3%、46.7%、23.3%,其差异均具有统计学意义(P〈0.05)。结论超声雾化吸入疗法较好地协助鼻咽癌放疗后治疗,可安全有效促进耳鼻咽喉及鼻咽黏膜修复,减少并发症发生率,是较理想的治疗方法。

Clinical value of ultrasonic atomizing inhalation in the prevention of otorhinolaryngological complications after radiotherapy in patients with nasopharyngeal carcinoma

Abstract:Objective To investigate the clinical value of ultrasonic atomizing inhalation therapy in prevention of complications after radiotherapy in patients with nasopharyngeal carcinoma. Methods 60 nasopharyngeal carcinoma patients with radiotherapy from March 2010 to March 2013 were randomly selected and divided into experimental group(30 cases) and control group(30 cases), with whom ultrasonic atomizing inhalation therapy and normal saline nasal irrigation(2 times/d for 3 months) were used respectively. And then the reactions of nasal mucosa and complications were compared. Results The rate of mild reactions(0~Ⅰ degrees) in the experimental group was 40%,and severe reactions(Ⅱ-Ⅳ degree) was 60%, And those in the control group were 10% and 90% respectively. Itshowed lighter reactions in the experimental group with a statistical difference(P〈0.05). The complication rate of throat dryness, otitis media with effusion, nose-sinusitis, nasal adhesions, choanal stenosis in experimental group was23.3%, 26.7%, 6.7%, 3.3%, 3.3%, respectively, which were significantly lower than in the control group, which were76.7%,80.0%, 53.3%, 46.7%, 23.3%, respectively(P〈0.05). Conclusion Ultrasonic atomizing inhalation therapy is a better method following with radiotherapy for its safe and effective repairing of otolaryngology and nasopharyngeal mucosa and less incidence of complications.
