改良环咽吻合术25例疗效分析

摘要：目的 回顾13年来对喉声门癌的改良环咽吻合术的远期疗效，研究这一手术的可行性和保证患者生活质量的效果。方法：1992年-2001年行改良环咽吻合术25例，男23例，女2例，所有患者术前均常规行MRI，CT及纤维喉镜检查。 结果：25例患者中随访3年时有21例存活，1例死于肿瘤复发，2例死于全身其它疾病，1例失访，3年生存率84%（21/25）。随访5年有18例存活，5年生存率72%（18/25）。术后拔管率96%（24/25）。患者术后均恢复了正常吞咽功能，且喉的发音功能理想。结论：改良环咽吻合术适合部分晚期声门型喉癌，该术式既能彻底切除肿瘤，又重建喉功能，提高了生活质量，值得推广。

Values observation of twenty-five patients undergoning modified cricoidopharynal anastomosis

[Absract] objiective: To respectively evaluate the curative and functional consequences of modified cricoidopharynal anastomosis. Methods:From 1992 to 2005, 25 patients underwent modified cricoidopharynal anastomosis, incluing twenty-three males and tow females . Before operation, all patients undergo MRI, CT and laryngofiberscope checking. Results: Overall three-year survival rate was 84%(21/25), overall five-year survival rate was 72%(18/25). All patients resumed normal oral feeding. 96%(24/25)patients achieved extubation . Respiratory and speaking function were good . Conclusion: Partialy glottal laryngocaicinoma patients are suited to undergo modified cricoidopharynal anastomosis. It not only improved local cancer control, but also preserve the function of laryngeal physiologic function.
