前置胎盘终止妊娠方式的选择

对我院1983年1月至1990年12月72例前置胎盘患者终止妊娠的方式及其结果进行分析。48例剖宫产,24例经阴道分娩。其中22例中央性前置胎盘的全部,9例部分性前置胎盘中的8例,41例低置胎盘中的18例行剖宫产。二种分娩方式均无产母死亡。围产儿死亡率剖宫产为83‰,经阴道分娩为80‰,二者比较,无显著性差异。本文认为,剖宫产可在短时间内结束分娩和制止出血,是前置胎盘患者终止妊娠的主要方法。对产前出血少,无其他合并症者,在严密监测下,阴道分娩亦为安全可行的分娩方式。

Choice of Mode of Delivery in Placenta Praevia

The delivery methods and the outcome of 72 cases of placenta praevia treated inthis hospital from Jan. 1983 to Dec 1990 were analyzed. Fourty-eight were deliveredby caesarean section (CS), 24 by vaginal delivery. All of the 22 cases of complete pl-acenta praevia, 8 of the 9 cases of partial placenta praevia and 18 of the 41 cases oflow lying placenta were delivered by caecarean section. There was no maternal denthin any kind of the delivery methods. The perinatal mortality in the cases by CSwas 83% and in vaginal delivery 80% showing no significant differences betweenthem. The results showed that caesarean section was the major delivery meteod in pla-centa praevia because it could terminate pregnancy and control hemorrhage promptly.Under close monitoring, the vaginal deliverg was also safe method if the hemorrhagewas of minor degres and the patient had not any complication.
