声波手机联合镍钛器械用于老年人下颌第二磨牙C形根管疗效观察

目的 观察声波手机联合镍钛器械应用于老年人下颌第二磨牙C形根管疗效.方法 选择上海市徐汇区牙病防治所2012年3月-2013年8月诊治的需行下颌第二磨牙C形根管治疗的患者150例(159颗患牙),分为观察组77例(81颗患牙)与对照组73例(78颗患牙),对照组采用手用不锈钢铿以逐步后退法进行根管预备;观察组先采用15号手用不锈钢锉建立引导通道,后予声波手机联合镍钛机动根管锉进行根管预备,两组均采用氢氧化钙糊剂进行根管充填.记录治疗期间疼痛反应与并发症发生情况,并于治疗后半年、1年、2年进行疗效评定.结果 观察组治疗期间疼痛反应显著轻于对照组,根管预备并发症发生率显著低于对照组,根管充填质量优于对照组(P＜0.05);治疗后半年观察组临床疗效优于对照组(P＜0.05);治疗后1、2年两组临床疗效差异无统计学意义(P＞0.05).结论 声波手机联合镍钛器械应用.于老年人下颌第二磨牙C形根管治疗可以减少疼痛及并发症,并提高根管充填质量和短期临床疗效.

Effect Observation of Sonic Combined with Nickel-titanium Endodontic Instruments in Application of Elderly Patients with C-shaped Root Canals Treatment in Mandibular Second Molars

Objective To investigate the clinical efficacy of sonic combined with nickel-titanium endodontic instruments in application of elderly patients with C-shaped root canals treatment in mandibular second molars.Methods A total of 150 patients (159 tooth),who would undergo C-shaped root canal treatment in mandibular second molar during March 2012 and Augest 2013,and were divided into observation group (n =77,81 tooth) and control group (n =73,78tooth).The root canals in control group were prepared by K-files,and the root canals in observation group were prepared by sonic combined with nickel-titanium endodontic instruments after establishing pathway with K-files.All root canals were filled with calcium hydroxide paste.Intraoperative pain reaction and incidence rate of complications were recorded,and curative effects were evaluated in 6 months,1 and 2 years after the operation.Results the intraoperative pain reaction was significantly milder (P ＜ 0.05),and the incidence rate of complications in root canals preparation was significantly lower than those in the control group,and the canal obturation quality was superior to that in control group (P ＜0.05);in observation group,the curative effect in 6 months after the operation was better (P ＜ 0.05),but there were no statistically significance differences in 1 and 2 years after the operation than those in control group (P ＞ 0.05).Conclusion The sonic combined with nickel-titanium endodontic instruments in application of elderly patients with C-shaped root canals treatment in mandibular second molars may decrease the postoperative pain and incidence rate of complications,and improve the canal obturation quality and short-term clinical efficacy.
