新生儿脐带血葡萄糖-6-磷酸脱氢酶筛查检测结果分析

摘    要:目的探讨新生儿脐带血葡萄糖-6-磷酸脱氢酶(G6PD)活性在新生儿G6PD缺乏症早期诊断中的意义。方法选择新生儿4625例,采用全自动生化分析仪对新生儿脐带血进行G6PD活性定量检测,并与新生儿自身静脉血进行对比,根据说明书参考值判定G6PD是否缺乏。结果 本组新生儿脐带血G6PD轻度缺乏症检出率为6.07%高于静脉血的5.36%,差异有统计学意义(P<0.05)。脐带血G6PD缺乏症总检出率为9.23%,静脉血为8.41%,两者比较差异无统计学意义(P>0.05)。脐带血与静脉血样本中均检测为G6PD缺乏症的384例患儿中,男性患儿检出率为83.33%高于女性的16.67%,差异有统计学意义(P<0.01)。结论新生儿脐带血取材方便、简单,可早期有效检测G6PD缺乏症,开展新生儿脐带血G6PD筛查,可对G6PD缺乏症引起的新生儿黄疸尽早明确诊断并及时治疗,且对降低新生儿高胆红素血症的发生率具有重要意义。

Abstract:Objective To explore the meaning of the detection of newborn umbilical cord blood G6PD in diagnosing newborn G6PD deficiency through the detection of of newborn umbilical cord blood G6PD activity.Methods 4625 newborns were chosen.The quantitative detection of umbilical cord blood G6PD activity was detected by automatic biochemical analyzer,which was compared with that of the newborn′s own venous blood.Whether G6PD was deficient was determined according to the instructions reference value.Results In this group,the mild G6PD deficiency detection rate of umbilical cord blood was 6.07% higher than 5.36% of venous blood,and the difference was statistically significant(P0.05).The total G6PD deficiency detection rate of umbilical cord blood was 9.23%,while that of venous blood was 8.41%,and the difference was of no statistical significance(P0.05).In 384 cases that were detected to be G6PD deficiency by both umbilical cord blood and venous blood samples,the detection rate of male was 83.33% higher than 16.67% of female,and the difference was statistically significant(P0.01).Conclusion It is convenient and simple to get umbilical cord blood,and umbilical cord blood can be used for the detection of G6PD deficiency effectively and early.To develop newborn umbilical cord blood G6PD screening can make neonatal jaundice caused by G6PD deficiency early diagnosed and treated timely,which is also of great significance for reducing the incidence rate of hyperbilirubinemia.
