经口机器人激光手术治疗早期声门型喉癌的疗效分析

摘    要:目的 探讨新型光纤CO2激光技术联合经口机器人手术治疗早期声门型喉癌的可行性及临床效果.方法 选取100例早期声门型喉癌患者作为研究对象,采用随机数字表法将其分为试验组和对照组,每组50例.试验组患者接受新型光纤CO2激光技术联合经口机器人手术治疗,对照组患者接受经口光纤CO2激光技术治疗.统计两组患者的手术时间、术中出血量、术后并发症和术后住院时间,并对两组患者进行为期2年的随访,比较两组患者的术后复发率.结果 试验组患者的手术时间明显短于对照组(P﹤0.01),术中出血量也明显少于对照组(P﹤0.01);两组患者的术后并发症发生率和复发率比较,差异无统计学意义(P﹥0.05);试验组患者的术后住院时间短于对照组(P﹤0.05).结论 达芬奇机器人外科手术系统具有更加稳定、操作灵活和视野清晰立体的特点,可弥补光纤CO2激光技术的不足,两者联合治疗早期声门型喉癌的治疗效果值得肯定,但应注意把握经口机器人手术的适应证,合理应用.

Analysis of the effect of transoral laser surgery in the treatment of early-stage glottic cancer
Abstract:Objective To investigate the feasibility and clinical effect of fiber CO2 laser combined with transoral ro-botic surgery in the treatment of early-stage glottic carcinoma. Method 100 patients with early-stage glottic cancer were randomized as study group or control group as per a random number table, with 50 cases in each group. The study group was treated with a new type of fiber CO2 laser combined with transoral robotic surgery, while the control group was ad-ministered by transoral fiber CO2 laser treatment. The operative time, intraoperative blood loss, postoperative complica-tions and hospital stay of these patients in the two groups were analyzed, and the patients were followed for two years, the recurrence rate in the two groups was compared. Result The operative time in study group was shorter than that in the control group (P<0.01), and so did the intraoperative blood loss (P<0.01);no significant difference was observed in re-gard of the incidence of postoperative complications and recurrence rate between the two groups (P>0.05), but shorter hospital stay was observed in study group as compared with control group (P<0.05). Conclusion The Da Vinci surgical system robot is characterized with robust performance, flexible operation and clear three-dimensional vision, which can make up for the shortcomings of CO2 laser technology, and the combination of the two techniques is favorable for the treatment of early-stage glottic cancer in consideration of the indications.
