摘    要:目的 探讨全肝CT灌注(CTP)成像对大肝癌患者经导管动脉化疗栓塞(TACE)治疗的临床价值.方法 选取采用TACE治疗的60例大肝癌患者作为研究对象,所有患者分别于治疗前、治疗后4周应用CTP,观察CTP参数的变化,参照RECIST实体瘤疗效评价标准,将60例患者分为有效组、无效组,对比CTP参数.结果术后4周,60例患者的CTP参数BF、BV、PS、MTT、HAF均较术前明显降低,差异均有统计学意义(P﹤0.01);CR患者8例(13.33%),PR患者34例(56.67%),SD患者13例(21.67%),PD患者5例(8.33%);治疗有效42例,无效18例.TACE治疗有效患者CTP参数BF、BV、PS、MTT、HAF均明显低于治疗无效患者,差异均有统计学意义(P﹤0.01).结论 CTP评价大肝癌患者TACE术后病灶变化及疗效具有重要的临床价值.

The clinical value of CTP in evaluating the efficacy of TACE treated patients with large hepatocellular carcinoma
Abstract:Objective To explore the clinical value of total hepatic CT perfusion (CTP) imaging in evaluating large hepatocellular carcinoma after transcatheter arterial chemoembolization (TACE). Method 60 patients with large hepato-cellular carcinoma (HCC) who were treated with TACE were included in the study. CTP were performed for all patients before and 4 weeks after treatment. The changes of CTP parameters were observed. According to RECIST, 60 patients were divided by with or without response to compare the CTP parameters. Result 4 weeks after TACE, the CTP parame-ters of all patients, including BF, BV, PS, MTT and HAF were significantly decreased compared to those before TACE (P<0.01);there were 8 cases (13.33%) of CR, 34 cases (56.67%) of PR, 13 cases (21.67%) of SD and 5 cases (8.33%) of PD;42 cases were with response, while 18 cases were not. Compared to those without response, the CTP parameters BF, BV, PS, MTT and HAF of those who achieved response were significantly lower, the difference was statistically signifi-cant (P<0.01). Conclusion CTP is vital in the evaluation of postoperative lesion changes of large hepatocellular carcino-ma after TACE.
