新生儿胆红素脑病MRI表现

目的:探讨新生儿胆红素脑病MRI表现及鉴别诊断,提高对本病的认识。方法:回顾分析19例新生儿胆红素脑病的影像及临床资料。结果:19例患儿中16例头颅MRI苍白球T1WI呈对称性高信号,T2WI呈等信号,扩散加权成像(dif-fusion weighted imaging,DWI)序列均未见信号改变。3例血清总胆红素(serum total bilirubin,TSB)值大于700 mol/L的患儿,双侧苍白球和腹侧丘脑T1WI均呈对称性稍高信号,其中2例急性期DWI序列未见异常,1个月后复查DWI呈高信号,苍白球T2WI出现高信号。1例未复查。结论:MRI-T1WI苍白球对称性高信号具有一定的特征性,DWI成像对本病的鉴别诊断有一定帮助。

The MRI features in acute with bilirubin encephalopathy in newborn infants

Objective:To discuss MRI features and differential diagnosis in bilirubin encephalopathy in order to improve the knowledge of this disease.Methods:Nineteen cases of bilirubin encephalopathy in newborn infants were retrospectively reviewed with regard to their MRI imagings and clinic data.Result:The 19 patients,16 cases of bilirubin encephalopathy were demonstrated symmetric,high-intensity signal in the globus pallidus on T_1WI and isointensity signal on T_2WI,there were no signal changes in DWI sequences.3 cases(TSB>700 mol/ L) were demonstrated high-intensity signal in the globus pallidus and subthalamus on T_1WI,of which two cases of acute stage were isointensity on T_2WI and no signal changes in DWI sequences.One month after the review there were high-intensity signal in DWI sequences and T_2WI.One patient was not reviewed.Conclusion:The symmetric,high-intensity signal in the globus pallidus on MRI-T_1WI is a characteristic finding in bilirubin encephalopathy.The DWI sequences have some help in the differential diagnosis of this disease.
