中医康复护理对肺癌术后患者肺功能及生活质量的影响

目的:探讨中医康复护理对肺癌术后患者肺功能及生活质量的影响。方法:选择2011年5月—2014年3月于我院就诊的肺癌术后患者89例,随机分为对照组44例和治疗组45例。对照组患者给予术后常规护理;治疗组在常规护理的基础上给予中医康复护理,1个月为1个疗程,护理3个疗程。观察患者术后乏力、恶心呕吐、疼痛、呼吸困难、纳少、失眠等临床症状的改善情况,用症状评分量表对证候进行评分;用由EORTC开发的QLQ-C30生活质量核心量表对患者的生活质量进行评价;检测患者的肺功能改善情况。结果:护理干预3个月后,治疗组患者术后乏力、疼痛、呼吸困难、纳少、失眠等临床症状的评分明显低于对照组患者,其差异有统计学意义(P<0.05);经护理干预后治疗组患者与生活质量密切相关的躯体功能评分、认知功能评分、角色功能评分、社会功能评分、情绪功能评分较对照组明显升高,其差异有统计学意义(P<0.05);治疗组患者经护理干预后的与肺功能密切相关的肺活量(VC)、深吸气量(TLC)、用力肺活量(FVC)、最大通气量(MVV)均有明显高于对照组患者,其差异有统计学意义(P<0.05)。结论:中医康复护理能有效缓解患者术后不适症状,改善患者肺功能,提高患者的生活质量,利于患者术后恢复,是临床术后治疗不可缺少的重要组成部分。

Effect of Traditional Chinese Medicine Rehabilitation Nursing on Lung Function and Living Quality of Lung Cancer Postoperative Patients

Objective：To investigate the effect of traditional Chinese medicine rehabilitation nursing on the lung function and living quality of the lung cancer postoperative patients. Methods ：89 lung cancer postoperative cases were randomly divided into a control group（44 cases）and a treatment group （45 cases）from May 2011 to March 2014. Patients in the control group were trea- ted with conventional nursing intervention while patients in the treatment group were treated with traditional Chinese medicine re- habilitation nursing, one month for a course, totally three courses for treatment. We observed the improvement of clinical indis- posed symptoms, mainly including fatigue, nausea, vomiting, pain, insomnia, difficulty breathing and decreased appetite and so on. We evaluated the symptom score and life quality score and observed the improvement of lung function. Results ： The clinical symp- toms scores of the patients in treatment group such as fatigue, nausea, vomiting, pain, insomnia, difficulty breathing and decreased appetite were higher than those of the control group and the difference was significant （ P 〈 0. 05 ）. The life quality scores such as physical function evaluation, cognitive function score, role function evaluation score, social function score and emotional function score were improved clearly and the degree of change in the treatment group was more pronounced and the difference was signifi- cant （P 〈 0. 05）. The levels of closely related pulmonary functions, such as VC ,TLC, FVC and MVV were improved clearly and the degree of change in the treatment group was more pronounced and the difference was significant （ P 〈 0. 05 ）. Conclusion ： The TCM rehabilitation nursing can effectively alleviate the uncomfortable symptom after surgery and improve the pulmonary function. It is beneficial to the postoperative recovery and an important part of clinical postoperative treatment.
