四瓣法功能性双侧唇裂修复术

目的：对双侧唇裂修复手术方法进行改进,以改善术后前唇组织薄弱、平坦及唇中部唇红不丰满等缺陷。方法：设计四瓣,用前唇上部内侧粘膜瓣（A瓣）加深唇龈沟,用下方的粘膜及红唇瓣（B瓣）重建唇珠,用两侧唇的皮肤粘膜瓣（C、D瓣）衬垫于前唇内侧以加固、加厚前唇;并将两侧唇肌肉缝合并固定于前唇上。结果：18例均获良好效果,术后上唇丰厚而无过紧、过短及塌陷畸形,唇红丰满美观辰珠形态好,动态畸形矫不明显。结论：此改进后的术式主要适合前唇前突严重者。

The function of cheiloplasty with four- flaps in bilateral cleft lip

Objective To improve deformity operation method of harelip, a new operative technique of cheiloplasty was used in bilateral cleft lip. Methods Four skin flaps and mucocutuneaus flaps were formed and used to repair cleft lip. "A" flap was used for prolonging anterior lip and "B" flap for reconstructing labial tuerbe of upper lip. "C" and "D" flaps was used for increasing thickness of anterior lip and preventing disruption of wound. Results The result shown that 18 cases of harelip obtained good treatment results. condlution The new operative method of cheiloplasty with four-flapsis was one of the best methods for repairing bilateral cleft lip.
