心房颤动合并心力衰竭患者临床分析

目的:通过对房颤合并心衰患者的临床分析,初步探讨其发病特点及诊治现状.方法:对90例房颤合并心功能不全患者进行回顾性分析,观察抗心律失常药物的疗效及并发症的情况,计算超敏CRP及INR基线值的平均值.结果:(1)高血压病,冠心病为其主要并发症;(2)予西地兰静推16h后房颤转复率为17%,心室率控制有效率为55%,使用可达龙及β受体阻滞剂后房颤转复及心室率的控制明显改善;(3)超敏CRP平均值明显升高;未用华法林患者INR基线值平均值低于房颤引起栓塞的最低INR值1.5.结论:对房颤合并心衰患者选用洋地黄类抗心律失常作用有限;需强调运用华法林抗凝;炎症是患者症状加重的影响因素.

Clinical Data Analysis of Patients with Atrial Fibrillation and Heart Failure

Objective: To primarily investigate the clinical aspects and current therapeutic status in patients with atrial fibrillation and heart failure by retrospective analysis.Method: Review the clinical data of 90 patients with atrial fibrillation and heart failure,evaluate the effects of regular anti-arrhythmia therapy and complications,meanwhile,detect the hypersensitive CRP level in all patients and INR level in patients without warfarin treatment.Results:(1)Hypertension and CAD are the major complications in these patients;(2)The recurrence of atrial fibrillation were 17% and the controlling of rapid ventricular rate was 55% 16 hours after received an intravenous dose of Cedilanid,amiodarone and beta-adrenergic blockers management will improve these indexes.(3) The average level of hypersensitive CRP in these patients was significantly increased and the average INR baseline in patients without warfarin treatment was lower than the minimum INR value in embolism caused by atrial fibrillation.Conclusion: The clinic effect of anti-arrhythmia drug of digoxin was limited in patients with atrial fibrillation and heart failure,warfarin treatment should be emphasized in these patients,and the inflammation will be one of important factors in the disease aggravation.
