神经肌肉电刺激治疗对脑梗死吞咽障碍患者情感障碍的疗效

目的 探讨神经肌肉电刺激(NES)治疗对脑梗死吞咽障碍患者情感障碍的影响.方法 选取有吞咽障碍的脑梗死患者126例,随机分为治疗组(66例)和对照组(60例),均给予常规药物治疗及吞咽训练,治疗组加用NES治疗,对两组患者治疗前后均进行症状自评量表(SCL-90)测试.结果 经过平均2个疗程的分组治疗,与治疗前比较,治疗组SCL-90中躯体化(23.65±9.28)分vs(33.45±10.61)分、强迫症状(18.56±6.25)分vs(27.73±8.17)分、人际关系敏感(20.27±8.12)分vs(24.61±8.27)分、抑郁(28.50±10.95)分vs (34.02±12.71)分、焦虑(21.50±7.37)分vs (28.68±8.23)分、敌对(12.18±5.64)分vs (16.91±5.35)分、精神病性(14.98±3.15)分vS(24.88±9.35)分等均有不同程度改善(P＜0.01),治疗后与对照组比较,治疗组躯体化、强迫症状、人际关系敏感、焦虑、敌对、恐怖、精神病性评分等改善明显(P ＜0.05或＜0.01).结论 NES治疗可以改善脑梗死吞咽障碍患者的情感障碍.

Effect of neuromuscular electrical stimulation on emotional disorders in patients of cerebral infarction with swallow dysfunction

Objective To study the effect of neuromuscular electrical stimulation（NES） to emotional disorders in patients with cerebral infarction complicated with swallow dysfunction.Methods A total of 126 cerebral infarction patients with swallow dysfunction were randomized into treatment group（n =66） and control group（n =60）.Patients in two groups received conventional drug therapy and swallow function training,while the patients in the treatment group received NES in addition.The patients of two groups were evaluated with SCL-90 scores before and after treatment.Results After treatment for two courses,somatic disorder （23.65 ± 9.28） scores vs （33.45 ± 10.61）scores,obsessive disorder （18.56±6.25） scores vs （27.73±8.17） scores,interpersonal sensitivity （20.27±8.12） scores vs （24.61±8.27） scores,depression （28.50± 10.95） scores vs （34.02± 12.71） scores,anxiety （21.50± 7.37） scores vs （28.68±8.23） scores,hostility（12.18±5.64） scores vs （16.91±5.35） scores,psychoticism（14.98±3.15） scores vs （24.88±9.35） scores of the treatment group in SCL-90 were all improved（P ＜0.01） than those before treatment.In comparison with control group after treatment,the following factors of treatment group improved more significantly,as somatic disorder,obsessive disorder,interpersonal sensitivity,anxiety,hostility,horror,psychoticism （P ＜ 0.05 or ＜0.01）.Conclusion NES applied in patients of cerebral infarction complicated with swallow dysfunction could improve emotional disorders.
