连续血液净化治疗ICU中多器官功能障碍综合征患者的疗效及预后分析

目的观察连续血液净化治疗ICU中多器官功能障碍综合征患者的疗效及预后相关影响因素。方法选择31例ICU中多器官功能障碍综合征患者，采用连续性血液净化治疗，治疗期间动态监测患者的血压、心率及血氧饱和度等指标，同时对患者治疗前后肾功能、电解质及酸碱等相关指标进行测定并比较。结果本组31例患者治疗前后血压、心率等无明显变化，治疗后K^＋、尿素氮（BUN）、肌酐（Cr）及HCO3^-较治疗前有显著变化，差异有统计学意义（P〈0．05）；31例患者中，18例存活患者和13例死亡患者的年龄、器官衰竭数目及APACHEⅡ评分差异均有统计学意义（P〈0．05）。结论连续血液净化治疗ICU中多器官功能障碍综合征可显著改善患者体内酸碱及水电解质紊乱，给患者救治提供了良好的基础，赢得了救治时间，患者预后与年龄、器官衰竭数目及APACHEⅡ评分等因素有关。

Analysis of curative effects and prognosis of continuous blood purification in treatment of patients with multiple organ dysfunction syndrome in ICU

Objective To observe the curative effects and prognosis of continuous blood purification in the treatment of patients with multiple organ dysfunction syndrome in ICU.Methods Thirty-one patients with multiple organ dysfunction syndromes in ICU received continuous blood purification treatment,during which the patients’ blood pressure,heart rate,oxygen saturation and other indicators were monitored dynamically.Meanwhile,the renal function,electrolyte,acid-base and other related indicators before and after treatment were measured and compared.Results Between before and after the treatment,the blood pressure,heart rate,etc did not change significantly,but K+,BUN,Cr and HCO3-changed significantly,with statistically significant difference(P < 0.05).Of the 31 patients,18 survived patients and 13 dead patients were significantly different in the patients’ age,the number of failed organs and the APACHEⅡ score,with statistically significant differences for all(P < 0.05).Conclusion Continuous blood purification therapy can significantly improve the acid-base and water electrolysis disorders of patients with multiple organ dysfunction syndrome in ICU,provide a good basis for treatment and win the time of treatment.Patients’ prognosis is related to the age,the number of failed organs,the APACHEⅡscore and other factors.
