同步化疗联合后程强调放疗治疗中晚期鼻咽癌的临床研究

目的评估中晚期鼻咽癌放射治疗联合同步化疗的临床效果及安全性。方法 200例中晚期鼻咽癌患者,随机分为对照组及研究组,均采用分次调强放射治疗,研究组联合顺铂（diamminedichloroplatinum,DDP）＋5-氟尿嘧啶（5-Fluorouracil,5FU）方案化疗,比较两组患者近期疗效、远期生存率、生存时间及放化疗毒副反应发生情况。结果研究组治愈率（cure rate,CR）为46.0%,对照组CR为34.0%,研究组CR高于对照组,研究组缓解率（remission rate,RR）为96.0%,对照组RR为87.0%,研究组RR高于对照组,研究组5年生存率为48.0%,对照组5年生存率为33.0%,研究组5年生存率高于对照组,研究组平均生存时间为（3.60±0.57）年,对照组平均生存时间为（2.90±0.39）年,研究组生存时间长于对照组,两组放射治疗副反应等级无显著差别。结论调强放疗联合同步化疗治疗中晚期鼻咽癌能够改善患者预后,延长生存时间,不加重放疗毒副反应,具有较高的临床安全性。

Clinical analysis of emphasis radiotherapy combined homeochronous chemotherapy on metaphase and advanced nasopharyngeal carcinoma

Objective To evaluate clinical effect and safety of emphasis radiotherapy combined homeochronous chemotherapy on metaphase and advanced nasopharyngeal carcinoma.Methods 200 cases patients with metaphase and advanced nasopharyngeal carcinoma were randomly divided into control groups and observation groups,all patients received fractional emphasis radiotherapy,patients in observation groups received homeochronous chemotherapy with diamminedichloroplatinum(DDP) combined 5-Fluorouracil(5-FU) in addition,curative effect in the near future,long-term survival rate,live time and toxic effect of radiotherapy were contrasted.Results CR was 46.0% in observation groups and was 34.0% in control groups,CR was higher in observation groups,RR was 96.0% in observition groups and was 87.0% in control groups,RR was higher in observation groups than in control groups,survival rate in 5 years were 48.0% in observation groups and was 33.0% in control groups,survival rate in 5 years was higher in observation groups than in control groups.live time was(3.60±0.57) years in observation groups and was(2.90±0.39) years in control groups,live time was longer in observation groups than in control groups,grading of radiotherapy toxic effect was indentical between therapia groups and control groups.Conclusion Emphasis radiotherapy combined homeochronous chemotherapy can improve prognosis on metaphase and advanced nasopharyngeal carcinoma,length live time,and is no effect to radiotherapy toxic,is safety in clinic.
