传统开颅手术治疗和小骨窗微创开颅对高血压脑出血患者的疗效及预后的影响

目的探讨传统开颅手术和小骨窗微创开颅术在高血压脑出血中的应用及效果。方法选择我院2011年1月～2012年1月收治的高血压脑出血患者60例,按照治疗方式的不同分为观察组和对照组,每组各30例,观察组采用小骨窗微创手术治疗,对照组采用传统开颅手术治疗,比较两组的疗效及预后情况。结果两组患者的残留血肿量、术后7 d的格拉斯哥（GCS）评分差异均无统计学意义（均P〉0.05）,观察组的手术时间及格拉斯哥预后评分（GOS）良好率均显著优于对照组,且观察组的再出血情况、生活能力及神经缺损情况等方面均优于对照组,差异均有统计学意义（均P〈0.05）。结论小骨窗微创开颅治疗高血压脑出血可显著提高疗效,改善患者的预后。

The effect of craniotomy operation and keyhole minimally invasive operation on curative efficacy and prognosis in hypertensive cerebral hemorrhage patients

Objective To investigate the application and curative efficacy on traditional craniotomy operation and keyhole minimally invasive operation in hypertensive cerebral hemorrhage patients.Methods 60 cases of hypertensive cerebral hemorrhage patients were selected in our hospital from January 2011 to January 2012 and divided into the observation group and the control group with 30 cases in each group according to the treatment modalities.Patients in observation group were treated with keyhole minimally invasive operation treatment,patients in control group were received traditional craniotomy operation,curative efficacy and prognosis were compared.Results The differences of residual hematoma volume and 7 d GCS score were all not statistically significant(all P>0.05),the operation time and excellent rate of GOS in ob-servation group were all better than those in control group,the situation of rebreeding,life ability and nerve defect degree were all better than those in control group(all P<0.05).Conclusion Minimally invasive craniotomy in treatment of hyper-tensive cerebral hemorrhage can significantly improve the curative efficacy and the prognosis.
