高频超声在尺神经肘部卡压病变诊断中的应用

摘    要:目的探讨高频超声在尺神经肘部卡压病变诊断中的应用价值。方法对171例尺神经肘部卡压患者的患肢与对侧的健肢肘部行高频超声检查,测量并比较两组尺神经的横径、前后径及横截面积(CSA)。观察患肢组尺神经超声图像变化,并将患肢组尺神经超声结果与神经电生理检查结果进行比较。结果患肢组尺神经的横径、前后径及CSA明显大于健肢组,两组比较差异有高度统计学意义(P〈0.01);患肢组尺神经的CSA与运动传导速度(MCV)呈负相关(r=-0.894,P〈0.01);高频超声对于判断尺神经损伤比电生理更早。结论高频超声检查对尺神经肘部卡压病变的诊断具有重要形态学参考价值。

Abstract:Objective To explore the value of the high-frequency ultrasound in the diagnosis of compression syndrome of ulnar nerve in the region of elbow.Methods Suffered limb and healthy limb of 191 cases with compression syndrome of ulnar nerve in the region of elbow were tested with high-frequency ultrasound and the diameter,anteroposterior diameter and cross-sectional area(CSA) of ulnar nerve were measured and compared between the two limbs.Ultrasound image changes of suffered limbs were observed and compared with neural electrophysiological test results.Results Ulnar nerve diameter,anteroposterior diameter and CSA of the suffered limb group were significantly greater than those of the healthy limb group(P < 0.01);CSA of ulnar nerve was negatively related to motor conduction velocity(MCV) in the suffered limb group(r =-0.894,P < 0.01).High-frequency ultrasound detection of ulnar nerve injury was earlier than electrophysiology.Conclusion The high-frequency ultrasound has important morphological reference value in the diagnosis of compression syndrome of ulnar nerve in the region of elbow.
