螺旋CT在结肠癌术前诊断和分期中的价值分析

目的探讨多层螺旋CT在结肠癌术前诊断和分期中的价值。方法回顾性分析我院2011年1~12月收集的结肠癌患者57例,将CT影像学分期与病理学分期结果相比较,观察多层螺旋CT在结肠癌术前诊断和分期中的准确性。结果多层螺旋CT对结肠癌T、N、M及TNM分期总的准确性分别为91.23%、84.21%、98.25%和70.17%。经一致性检验分析,多层螺旋CT对结肠癌T、N及TNM分期的准确性与术后病理分期结果比较,两者诊断一致性较好,差异均无统计学意义（P〉0.05）,仅1例患者有远处转移,并于随诊中确诊。结论多层螺旋CT对结肠癌术前定位和肿瘤侵犯范围的判定有一定的应用价值,对淋巴结的转移可提供一定的参考,值得推广应用。

Analysis of preoperative diagnostic and staging value of spiral CT for colorectal carcinoma

Objective To explore preoperative diagnostic and staging value of multislice spiral CT for colorectal carcinoma.Methods 57 patients with colorectal carcinoma in our hospital from January to December 2011 were retrospectively analyzed.The preoperative multislice spiral CT imageology staging results were compared with the postoperative pathological staging,and the accuracy rates of preoperative diagnostic and staging by multislice spiral CT were observed.Results The accuracy rate of multislice spiral CT for colorectal carcinoma at T,N,M and TNM staging was 91.23%,84.21%,98.25%,70.17% respectively.The consistency test showed that the accuracy of colorectal carcinoma T,N,M and TNM staging by multislice spiral CT had good consistency with postoperative pathological staging,there was no significant difference(P > 0.05).Only 1 patient had distant metastasis,and made a definite diagnosis during follow-up.Conclusion Multislice spiral CT has a certain value in preoperative localization and tumor invasion of colorectal carcinoma,which also provide some reference value for the lymph node metastasis,and should be widely applied.
