标准大骨瓣开颅减压术治疗重型颅脑损伤的疗效观察

目的探讨标准大骨瓣开颅减压术（SLTC）治疗重型颅脑损伤（STBI）的临床效果。方法将138例STBI患者随机分为两组，观察组69例采用SLTC治疗，对照组69例采用常规骨瓣开颅减压术治疗，比较两组的疗效。结果观察组颅内压下降速度和幅度明显大于对照组（P〈0．05或P〈0．01）；观察组神经功能缺损评分和术后并发症较对照组均明显降低（P〈0．05）；观察组术后预后较对照组优（P〈0．05）。结论SLTC具有视野开阔、减压充分、操作方便、能显著提高患者生存质量并减少病死率的优点，是一种既有效又安全的方法。

Observation of the curative effect on the standard large trauma craniotomy in the treatment of the severe traumatic brain injury

Objective To investigate the clinical effect of the standard large trauma craniotomy(SLTC) in the treatment of severe traumatic brain injury(STBI).Methods 138 patients with severe traumatic brain injury were randomly divided into two groups,69 cases in the observation group were treated with the standard large trauma craniotomy,and 69 cases in the control group were treated with the routine craniotomy.The curative effects of the two groups were compared.Results The decreasing rate and extent of intracranial pressure in the observation group were greater than those in the control group(P < 0.05 or P < 0.01);the NIHSS and complication in the observation group were lower than those in the control group(P < 0.05);the prognosis of the observation group was better than that in the control group(P < 0.05).Conclusion The SLTC has advantages of wide vision,full decompression,convenient operation,improvement of the patient’s life quality,decrease of the fatality rate,which is a safe and effective method.
