糖尿病患者心率变异与晨峰心率关系的探讨

目的探讨糖尿病患者心率变异性及其晨峰心率的变化特征。方法对30例糖尿病患者和30例非糖尿病患者分别行24 h动态心电图监测,分析其心率变异性时域指标及晨峰心率的变化。结果经秩和检验分析,两组间R-R间期的标准差（SDNN）、R-R间期平均值的标准差（SDANN）、R-R间期的平方根（RMSSD）、两个相邻R-R间期差值〉50 ms的心跳数所占分析信息期内心搏数的百分比（PNN50）各参数差异均有高度统计学意义（P〈0.01）。经方差分析,糖尿病组3个时间段心率最大值和最小值差异均无统计学意义（P〉0.05）,非糖尿病组3个时间段心率最大值和最小值差异有统计学意义（P〈0.05）。经t检验,糖尿病组和非糖尿病组患者在晨起的3个时间段内,只有〉6∶00～7∶00心率最小值间的差异无统计学意义（P〉0.05）。结论由于糖尿病患者心脏自主神经功能损害,心率变异各项指标降低,晨峰心率变化不明显。

Discussion of relationship of heart rate variation and morning peak heart rate in the diabetes patients

Objective To discuss heart rate variability and the change characteristics of morning peak heart rate of diabetes patients.Methods 30 cases of patients with diabetes and 30 cases of patients without diabetes were performed respectively the dynamic ECG monitoring in 24 hours to analyze the heart rate variability time domain indexes and morning peak change of heart rate.Results By the rank and test analysis,between the two groups,the differences of parameters SDNN,SDANN,R-MSSD,PNN50 were statistically significant(P < 0.01).By the analysis of variance,for the diabetes group,differences of heart rate of three time the maximum and the minimum were not statistically significant(P > 0.05),for the group of patients without diabetes,differences of three time heart rate the maximum and the minimum were statistically significant(P < 0.05).By t test,diabetes group and the group of patients without diabetes in the morning rises,three time,only heart rate differences in > 6∶00-7∶00 between the minimum value were not statistically significant(P > 0.05).Conclusion Due to the damage of cardiac autonomic nerve function of diabetes patients,indicators of heart rate variability reduce,the morning peak heart rate does not change significantly.
