前列地尔联合雾化吸入硝普钠治疗新生儿持续肺动脉高压的疗效观察

目的探讨前列地尔联合雾化吸入硝普钠治疗新生儿持续肺动脉高压的疗效。方法将58例诊断为新生儿肺动脉高压的患儿随机分为治疗组（28例）和对照组（30例）。对照组给予头罩加压吸氧、抗凝、强心、利尿等基础治疗及钙通道阻滞剂;治疗组在对照组治疗基础上联合前列地尔和雾化吸入硝普钠。在用药前、后分别测量肺动脉收缩压（PAP）、动脉血氧饱和度（SaO2）、血氧分压（PaO2）、血压（BP）和心律（HR）的数值。结果两组患儿治疗后在肺动脉压力、动脉血氧分压、血氧分压指标均有明显改善（P〈0.05）,治疗组较对照组改善更显著（P〈0.05）。结论前列地尔联合雾化吸入硝普钠是治疗新生儿肺动脉高压的有效方法。

Observation of curative effect on Alprostadil combined with aerosolized inhalation of Sodium Nitroprusside in the treatment of newborn with persistent pulmonary hypertension

Objective To explore the therapeutic effect of Alprostadil combined with aerosolized inhalation of Sodium Nitroprusside in the treatment of newborn with persistent pulmonary hypertension.Methods 56 newborns with pulmonary artery hypertension were randomly divided into control group(n = 30) and treatment group(n = 28).The control group was given hood pressurized oxygen,anticoagulation,myocardial contraction stimulating drugs,diuresis and calcium channel blockers;the treatment group was given Alprostadil combined with aerosolized inhalation of Sodium Nitroprusside on the basis of the control group.The dynamic changes of pulmonary arteries pressure(PAP),blood oxygen saturation(SaO2),blood oxygen partial pressure(PaO2),blood pressure(BP) and rhythm of the heart(HR) were measured before and at the end of therapy.Results After treatment,two groups of newborns in PAP,SaO2,PaO2 index were improved obviously(P < 0.05),although the treatment group was improved significantly than the control group(P < 0.05).Conclusion It is proved that an effective method to cure pulmonary hypertension for newborn is Alprostadil combined with aerosolized inhalation of Sodium Nitroprusside.
