导乐陪伴分娩对初产妇产时产后的作用分析

目的探讨导乐陪伴分娩对初产妇产时产后的作用。方法对采用导乐陪伴分娩的174例初产妇（观察组）与仅有家属陪伴分娩的180例初产妇（对照组）分娩的分娩结局进行比较。结果观察组总产程时间为（7．41±1．02）h，明显短于对照组（10．42±2.02）h，差异有统计学意义（t=-3．23，P〈0．05）；剖宫产率为10．92％，明显低于对照组14．44％，差异有统计学意义（x^2=6．33，P〈0．05）；新生儿窒息率为2．30％，明显低于对照组6．67％，差异有统计学意义（x^2=5．41，P〈0．05）；产后出血量（124．3±51．5）ml明显少于对照组（159．7±43．2）h，差异有统计学意义（t=2.35，P〈0．05）；泌乳始动时间（25．43±9．78）h，明显早于对照组（40．12±10．62）h，差异有统计学意义（t=-3．64，P〈0．05）。结论导乐陪伴分娩能促进和支持自然分娩，降低剖宫产率，有利于提高产科质量，保障母婴心理、生理健康和安全。

Effects of doula delivery on intrapartum and postpartum

Objective To investigate the effects of doula delivery on intrapartum and postpartum. Methods 174 cases of primip who accepted doula delivery were chosen as observation group and 180 cases of primip who were nursed only by family members were chosen as control group. The delivery outcomes of two groups were analysed and compared. Results The total accouchement time of observation group was obviously shorter than the control group （t=3.23, P〈0.05）. The cesarean section rate of observation group was significantly lower than the control group （X^2=6.33 ,P〈0.05）. The neonatal asphyxia rate of observation group was significantly lower than the control group （X^2=5.41, P〈0.05）. The amount of postpartum hemorrhage in observation group was less than the control group （t=2.35, P〈0.05）. The initial time of lactation in observation group obvious was earlier than the control group （t=3.64, P〈0.05）. Conclusions Doula delivery can promote and sustain spontaneous labour, also can degrade the rate of cesarean section. The system helps to improve the quality of obstetrics and ensure mental health, physical health and safety of mother and infant.
